第四节 眶尖端综合征
眶顶为由颅内进入眼球的神经和血管的通道。又是眼外肌和眼提上睑肌的起点。在眶顶部有眶上裂、眶下裂及视神经孔。
任何病变波及至眶顶部，便可出现眶尖端综合征。
一、病因
凡颅部及眶部外伤有颅骨或眶骨骨折伤及眶上裂、视神经孔，眶内肿瘤、眶内外伤或手术后的出血、血肿，或各种炎症如骨膜炎、血管周围炎等波及眶上裂及视神经孔，伤及第Ⅱ、Ⅲ、Ⅳ、Ⅵ脑神经及第Ⅴ脑神经的眼支者，均为其病因。
二、临床表现
临床表现除与眶上裂综合征相同外，因视神经被累及，因而还有视力障碍，视力可以减退或黑矇。
三、诊断
根据病史及临床表现、X光摄片、B超及CT检查可作出诊断。
四、治疗
按照不同的病因，进行不同的治疗。凡属炎症者，视病情轻重。可服用大剂量或中等剂量的强的松，效果较好。还可辅助应用抗生素如青霉素、新青Ⅰ，以及磺胺制剂。
