(Dept. of Gynaecology and Obstectrics, the 2nd Affiliated Hospital of C
hina Medical University)

Objective:To provide the morphological data of the vascular ba
lls of human dermal papillae for the research on skin damage and repair and related fields.

Methods:Skin blood vessels were infused with A.B.S. and the vascul
ar cast was observed with SEM.

Results:The morphological characteristics and formation mechan
ism of 6 kinds of dermal papillary vascular balls in human hands and feet were observed.

Conclusion:Except for the pin-like vascular loops in human skin der
mal papillae, there were a lot of vascular balls which existed in it and formed by some compiling vascular loops. This is a special and rich in blood supply area, and is worth further research.

A Preliminary Research on the Antiinflammatory and
Detoxifying Effects of MEBO
(Abstract)
Huang Qingshan Li Kaixian Zhu Danqing
(The 2nd People's Hospital, Laiwu City, Shandong Province)

Experiments were performed for investigating the effects of MEB
O on plasma interleukin-8(IL-8), nitrite ion and nitrate ion ratio, comp
lement 5a(C5a) and polymorphonuclear neutrophilic leukocyte(PMN) ch
emotaxis index(CI) in rabbits with endotoxin-induced disseminated int
ravascular coagulation(DIC). The results showed that plasma IL-8, nitr
ite ion and nitrate ion ratio, C5a and CI levels of PMN increased markedly in model group, which were confirmed pathologically with obvious damage of tissues or organs, while in the MEBO group, the above mentioned parameters and damage of tissues or organs were reduced significantly (P<0.01). The results suggested that the IL-8 and NO might be involved in thepathogenesis of endotoxin-induced DIC and MEBO could be used for preventing or treating DIC through a mechanism of regulation of cytokines network.

Staphylococcus Aureus Bacteremia of Burn Patients
--An Analysis of 179 Cases
(Abstract)
Zhang Hong Yang Xiaodong
(Burn Unit, the 159th Hospital, PLA, Zhumadian, Henan Province)

To find out the pathogenesis of bacteremia of burns and the measures for its prevention and treatment, a retrospective study of the analysis of 179 cases of Staphylococcus aureus bacteremia treated in our hospital from 1981 to 1995 was made. It was shown that bacteremia occurred in patients with an average TBSA of 52.39%, and third degree burn area 27.93%. The stage of high incidence was in two weeks post burn and in the perioperative period. The incidence and prognosis of bacteremia were related to burn area, inhalation injury, shock, agents causing bacteremiaor type of bacteria invasion, infective complications and so on. It was suggested that comprehensive prevention and treatment measures should be taken to control the development of bacteremia and efficacious an tibiotics should be adminstered for those who are highly susceptible to bacterial infection. This is the key to the reduction of the morbidity and mortality of bacteremia of burn patients.

The Application of Renal Functions Radioimmunoassay
in Serious Burns
(Abstract)
Yang Xianwen Wang Xinting
(Burn Research Institute, Hebei Province)

Renal functions radioimmunoassay is a sensitive method for diagnosing the changes of renal functions in the early stage of burn. The urine Alb, serum and urine β2 -m, IgG, and THP were serially determined with radioimmunoassay to assess the renal functions during various stages of burn in 71 patients with TBSA over 30% (X±S=60.40%±19.24%).It was found that the urine levels of Alb,IgG and THP and the serum and urine levels of β2 -m were remarkedly increased and that the serum levels of IgG were decreased after burns. The results of examining differential molecular weight proteins of urine may mirror the changes of glomerular permeability and degree of injury.Blood β2 -m change is a sensitive target evaluating glomerular filtration function, especially, when blood creatinine is in blind zone.Urine β2 -m and THP(mucin)RIA have the locating role in renal tubes injuries (proximal convoluted tube, distant convoluted tube).Renal functions radioimmunoassay method is easy toooperate, its blood and urinary samplings are convenient, the providing data are seriation, its credibility is high and is easy to popularize.

An Analysis of 84 Cases of Multiple System Organ
Failure Post Burn
(Abstract)
Sun Zhigang Qi Shunzhen Lin Hongwu Liu Jianchun
(Dept. of Burn Plastic Surgery, International Peace Hospital Named aft
er Norman Bethune)

A retrospective analysis of 84 cases of burn complicated by multiple system organ failure (MSOF) was made. It showed that MSOF is closely related to degree of burn, shock, inhalation injury and severe infecti on. Delayd type MSOF is directly related to shock and infection and kidney is initially attacked. Immediate type MSOF is closely related to inhalation injury and lung is initially attacked. The more the organs involved the higher the mortality. Measures for prevention and treatment of MSOF are presented.

treated with MEBO/MEBT. The smallest burn area was 50%BSA and the large
st 95%BSA. All the patients had deep second degree and third degree burns. 10 of them were complicated by moderate or severe inhalation injury.Timely measures including trachetomy, oxygen inhalation and aerosolinhalation were taken. All the patients were cured, except for one died from MSOF.

During operation under extracorporeal circulation, high frequency electric knife is used to excise the tissue. 12 patients were burned electrically The burn wounds were small and superficial. Most of the wounds located on prominent parts of the bone. Most of them were single. 41.7% of them were double.

The wounds were treated locally with tinctures and gentian violet.
The wounds were dry but healed with scar formation. The authors suggest
ed some measures for the prevention of burn caused by electric knife.

A case of extensive pyogenic ulcer in both shanks complicated by secondary iinfection, toxic shock and acute heart and kidney failure.
After timely systemic treatment, the symptoms were controlled. The wounds were then treated with MEBO. Interrupted exposure techniique was
used. The wounds healed without scar formation. The auther pointed out
that when applying MEBO, the doctors should not rigidly adhere to the ex
posed technique only. Exposed, semi-exposed and interruptedly exposed techniques should be appplied in a flexible way according to the case.

18 cases of soft tissue crush injury after early incision and drainage were treated with MEBO gauze. MEBO helped to promote early liquefaction of necrotic tissue, revivification of parabiotic tissue and growth of granulation tissue. It preserves parabiotic tissue to the greatest extent and markedly reduces disablement rate. Part of the large defects can be repaired by autotransplantation of skin or transposition of skin flap. The results were satisfactory.
