【摘要】 目的 探讨Ⅰb、Ⅱa期子宫颈癌根治术中髂内动脉灌注化疗的效果。方法32例宫颈癌患者，Ⅰb1期12例，Ⅰb2期6例，Ⅱa1期14例。在未施行广泛性子宫切除术+盆腔淋巴结清扫术前，先行结扎两侧髂内动脉5例，髂内动脉前干27例;在结扎线远侧分别从两侧动脉推注经溶化稀释的卡铂400mg/m 2 ，博莱霉素30mg，丝裂霉素8mg，最后推注长春新碱2mg，然后施行宫颈癌根治性切除术。术后1月再全身静脉化疗2～3个疗程。结果 术后除1例切口延期愈合外无其它手术并发症。随访5年以上者22例，5年生存率为90.9%。共复发2例，复发率为6.3%。结论 Ⅰb、Ⅱa期子宫颈癌根治术辅以髂内动脉灌注化疗，可有效预防术中盆腔内淋巴结转移，提高5年生存率。
