【摘要】 目的 探讨小骨窗开颅手术治疗高血压脑出血的方法和价值。方法 54例病人行小骨窗开颅手术治疗，其中壳核-内囊型18例，壳核进展型12例，壳核型14例，皮层型10例。术后第1、4、7天和1个月、3个月复查头部CT。结果 术中血肿完全清除10例，血肿清除90%以上18例，血肿清除70%～80%26例。疗效根据GOS标准，优10例，良16例，中10例，差7例，死亡12例。结论 对血肿量大于30ml，一般情况稳定，应积极早期行小骨窗开颅清除血肿。
