【摘要】 目的 研究医疗事故鉴定中首次鉴定结论错误的案例的人为现象和常见的原因。 方法 对61起医疗事故案例的相关资料进行回顾性分析。 结果 首次鉴定结论为“不属医疗事故”，或“无法鉴定”等，再次鉴定为医疗事故的12例:其中诊断错误的4例;医疗中操作失误的5例;医护管理缺陷的3例。 结论 对医疗事故案件的首次鉴定需提高质量，减少再次累鉴。
