【摘要】 目的 探讨肝动脉多普勒血流灌注指数(DPI)对肝硬化患者结节性病灶的鉴别诊断价值。 方法 选择79例肝硬化伴结节性病灶的患者，应用超声检测肝动脉DPI，以DPI≥0.30为诊断肝硬化结节性病灶为癌性结节的标准。 结果 79例患者中33例结节性病灶为癌病灶，其余46例为良性病灶;依据DPI诊断肝硬化结节性病灶为癌性结节的敏感性为90.9%、特异性为89.1%、准确性为89.9%、阳性预测值为85.7%、阴性预测值为93.2%。结论 DPI可作为临床诊断与鉴别诊断肝硬化结节良恶性的可靠参数。

    【Abstract】 Objective To study the identified diagnostic value of Doppler perfusion index(DPI)of hepatic artery by ultrasonography in the patients with liver cirrhosis.Methods Seventy-nine patients with liver cirrhosis with tubercular focus were selected.Their DPI of hepatic artery were measured by ultrasonography.The diagnostic cri-teria of hepatic carcinoma is that DPI is above0.30.Results The focuses of thirty-three out of seventy-nine pa-tients with liver cirrhosis were malignant.The focuses of the other forty-six patients were benign.The sensitivity of DPI as the diagnostic criteria of malignant focus was90.9%;the specificity was89.1%;and the accuracy was89.9%.The positive predictive value was85.7%;and the negative predictive value was93.2%.Conclusion It is a reliable index for DPI to be a criteria to diagnose malignant or benign focuses in liver cirrhosis. ......
