摘 要 为研究早期肠道喂养(EEF)对严重烧伤大鼠肝功能的保护及其作用机制,采用Wistar大鼠30%体表烫伤模型,动态观察了EEF对伤后胃粘膜内pH(pHi),门静脉内毒素水平,肝组织含水量,血浆肿瘤坏死因子(TNF-α)、丙氨酸转氨酶(ALT)、天冬氨酸转氨酶(AST)活性,总胆红素(TB)、直接胆红素(DB)含量,以及血浆总蛋白、白蛋白浓度变化的影响。结果显示,与烧伤对照组相比,EEF对严重烧伤大鼠肝功能损害具有明显的改善作用;其机制可能与增加内脏血流量、减少肠源性内毒素吸收及炎性介质的释放有关。

PROTECTIVE EFFECTS OF EARLY ENTERAL FEEDING ON THE HEPATIC FUNCTION AND THEIR MECHANISMS AFTER SEVERE BURNS IN RATS

Abstract In order to study the protective effects of early enteral feeding (EEF) on the hepatic function and their mechanisms after severe burns in rats, Wistar rats with 30% of total body surface area (TBSA)full-thickness burn were employed, the effects of EEF on the postburn changes of gastric intramucosal pH(pHi), endotoxin levels in portal vein, water contents of hepatic tissue, blood concentrations of tumor necrosis factor (TNF-α), plasma activities of alanine aminotransferase(ALT), asparate aminotransferase(AST), as well as the blood contents of total bilirubin(TB) and direct bilirubin(DB), total protein(TP) and albumin(ALB) were dynamically observed. The results showed that EEF had beneficial effects on the improvement of hepatic function in rats after severe burns, which was probably related with the increase of splanchnic blood flow, decrease of the absorption of gut-origin endotoxin and the liberation of inflammatory mediators.
