摘要 目的： 探讨高血压病心率变异性(HRV)的特点。方法： 根据超声心动图结果将60例高血压病患者按左心室构型分为正常构型组、左心室重构组和左心室肥厚组3组，每组各20例；对照组30例，对比分析其HRV的变化。结果： 与对照组比较，左心室肥厚组、左心室重构组HRV各参数均有显著性差异(P<0.05，P<0.01),正常构型组中仅有HF和LF/HF存在显著差异(P<0.05)；高血压病各组间HRV比较，左心室重构组与正常构型组间仅频域指标有差异(P<0.05，P<0.01),其余各组间HRV各参数均有显著性差异(P<0.05，P<0.01)。结论： 按高血压病左心室构型的差异分组研究HRV很有价值。依左心室不同构型HRV的变化，评估高血压病早期的自主神经功能状态，有望对高血压病的早期预防、诊断、治疗提供帮助。
ANALYSIS OF HEART RATE VARIABILITY IN HYPERTENSION WITH PATTERNS OF LEFT VENTRICULAR GEOMETRY

Abstract Objective: To study characteristics of heart rate variability(HRV) in hypertension.Methods: According to ultrasonic cardiography(UCG) findings,hypertension with patterns of left ventricular geometry were classified into the three groups:Group Ⅰ(20 patients with a normal left ventricle),Group Ⅱ(20 patients with left ventricular concentric remodelig),Group Ⅲ(20 patients with left ventricular hypertrophy) and Group Ⅳ(30 patients for control).The relationships of HRV between the groups were determined.Results: There was a great difference in the parameters of HRV between these four groups(P<0.05,P<0.01).High frequency(HF) and low frequency(LF)/HF were significantly different in Group Ⅰ from Group Ⅳ(P<0.05).The spectrum analysis indicated that there was a remarkable difference in HRV between Group Ⅲ and Group Ⅰ(P<0.05,P<0.01).The parameter of HRV was significantly different from each other in the other groups.Conclusion: According to hypertension with patterns of left ventricular geometry,to study HRV is very valuable.On the basis of left ventricular geometry variation of HRV,an evaluation of autonomic nerve activity of hypertension in the early stage may be helpful in the prevention,early diagnosis and early treatment of hypertension stage.
