摘要 目的:了解中国人胰岛素依赖型糖尿病(IDDM)患者谷氨酸脱羧酶(65 KD)抗体(GAD 65 A)的流行情况,探讨酶免疫法检测GAD 65 A的临床应用价值。方法:采用酶免疫法测定45例IDDM患者和41例正常对照者血清中GAD 65 A的含量,并与放射免疫沉淀法进行比较。结果:IDDM组GAD 65 A浓度和阳性率分别为212.95 ng/ml±433.09 ng/ml和44.44%,对照组分别为1.33 ng/ml±3.64 ng/ml和0,2组间存在显著差异(P<0.01)。酶免疫法和放射免疫沉淀法比较,两者GAD 65 A检出率无显著差异(56.3% vs 87.5%,P>0.05)。结论:中国人IDDM的发病机制与自身免疫密切相关。酶免疫法测定GAD 65 A对临床IDDM的诊断和预测具有应用价值。

    

DETERMINATION OF ANTIBODIES TO GLUTAMATE DECARBOXYLASE

Abstract Objective:To understand the frequency of antibodies to glutamate decarboxylase(65 KD)(GAD 65 A) in Chinese patients with insulin-dependent diabetes mellitus(IDDM) and to explore the clinical values of enzyme immunoassay for GAD 65 A.Methods:GAD 65 A were measured in sera from 45 patients with acute-onset IDDM and 41 healthy controls by using enzyme immunoassay,and this method was also compared with radioimmunoprecipitation assay.Results:The mean concentration and positive frequency of GAD 65 A were 212.95 ng/ml±433.09 ng/ml and 44.44% respectively in IDDM group,compared with those of 1.33 ng/ml±3.64 ng/ml and 0 respectively in the control group (P<0.01).There were no significant difference between the GAD 65-positive frequency detected by enzyme immunoassay and that by radioimmunoprecipitation assay (56.3% vs 87.5%, P>0.05).Conclusion:The pathogenesis of IDDM in Chinese is closely related to autoimmune disorder.The enzyme immunoassay may be used in diagnosis and prediction of IDDM.
