【摘要】 目的 探讨血管活性物质与细胞膜离子转运的关系及其在高血压病(EH)发病中的作用。方法 观察107例EH患者和30例正常人血浆多种血管活性物质、淋巴细胞离子泵和阳离子变化,并对血压与13个相关预选因子进行逐步回归分析。结果 EH患者血浆内皮素、血管紧张素Ⅱ、内源性洋地黄物质、去甲肾上腺素和心钠素增高,它们彼此相关并与淋巴细胞钠泵和钙泵活性下降、Na+ 和Ca2+ 浓度升高相关,其中血管紧张素Ⅱ、去甲肾上腺素、心钠素、细胞钠泵和Na+ 与血压之间有独立的相关关系。结论 体循环中多种血管活性物质含量改变和相互作用与细胞膜离子转运缺陷有密切的内在关系,并可能在EH的发病过程中起协同和促进作用。

Abstract Objective To sutdy the effectof vasoactive substances and cell cation transport function in the pathogenesis ofessential hypertension (EH). Methods Plasmaendothelin (ET), angiotensin (Ang ), endogenous digitalis-like substances (EDLS), norepinephrine (NE)and atrial natriuretic peptide (ANP) were determined, lymphocytic membraneion pumps andcations were also measured in 107 EH patients and 30 healthy subjects. Multiple stepewiseregression analysis was computed using blood pressure as dependent variable and 13parameters as independent variables. Results It wasfound that in EH patients, ET, Ang , EDLS, NE andANP were all increased and correlated with each other. The increase of ET, Ang , EDLS, NE and ANP were closely related to the decrease of Na+ -K+ -ATPaseand Ca2+ -ATPase activities and the enhance of Na+ and Ca2+ .The regression equation revealed that Ang , NE, ANP,Na+ -K+ -ATPase activity and Na+ were the independentfactors affecting blood pressure in EH patients. Conclusion The changes of vasoactive substances levels in the circulation andcation transport function in the cell, or the interaction among them may play importantroles in the pathogenic mechanisms of EH.
