【摘要】 目的 探讨冠状动脉钙化与造影所示冠状动脉狭窄间的关系。方法 对218例因临床怀疑或确诊为冠心病而行冠状动脉造影的患者,用电子束CT(EBCT)检查其冠状动脉钙化。结果 冠状动脉狭窄组的钙化总积分明显高于非狭窄组(P<0.001),二者积分分别为229±363,42±125。冠状动脉狭窄支数与钙化积分呈正相关。全组冠状动脉钙化预测冠心病的总体敏感性、特异性及准确性分别为82%、75%及79%,与老年组相比,年轻组的特异性较高而敏感性较低。结论 研究结果提示:EBCT钙化积分是预测冠心病的有价值的指标。

    Quantification analysis of coronary artery calcification byelectron beam computed tomography in Chinese ZhangShaoxiong, Dai Ruping, Gao Runlin, et al. Cardiovasclar Institute & Fu Wai Hospital,PUMC & CAMS, Beijing 100037

    【Abstract】 Objective To investigate therelationship of coronary artery calcification (CAC) and angiographically significantcoronary artery disease. Methods Electron beamcomputed tomography (EBCT) was performed for detecting coronary calcification in 218patients underwent coronary angiography whom were suspected coronary artery disease.Significant coronary artery stenosis was defined as greater than 50% narrowing of lumen.Results Among 135 patients in whom CAC wasidentified, 86% had angiographically significant coronary stenosis, and 82% of 142patients with angiographically significant coronary stenosis showed CAC by EBCT. The totalscores of CAC in patients with significant coronary artery disease (CAD) demonstrated byangiography were much higher than those in patients without CAD, the scores were 229±363,42±125, respectively (P<0.001). There was a positive relation between the number ofvessels with significant stenosis and the total calcification score, on the other hand,the more the vessels with CAC, the higher the probability of the patients with significantcoronary stenosis. The overall sensitivity, specificity and accuracy of CAC for stenosiswere 82%, 75% and 79% respectively. In the younger age group the sensitivity was lower andthe specificity was generally higher than that in the elderly group. The cutpoint ofcalcification score for prediciting significant stenosis for 40-49, 50-59 and 60-69 yearsage group in present study were 1,25 and 50 respectively. Conclusion Electron beam CT calcification score appears to be an effectivepredictor of coronary artery disease.
