乳腺肿瘤端粒酶活性检测的临床意义

摘要 目的 探讨端粒酶在良、恶性乳腺肿瘤中表达的差异以及它与恶性乳腺肿瘤的病理分型、临床分期之间的关系。方法 采用TRAP(telomericrepeated amplification protocol)-银染法检测保存于液氮中的新鲜组织标本中的端粒酶活性。结果 良、恶性乳腺肿瘤的端粒酶阳性率差异有显著性(P<0.005),但它们的瘤旁组织中的端粒酶活性皆为阴性。恶性乳腺肿瘤端粒酶的阳性率随病理分型的恶化和TNM分期的进展而逐渐增高。恶性乳腺肿瘤中肿瘤长径≥20mm及淋巴结转移阳性组的端粒酶阳性率显著高于<20 mm及淋巴结转移阴性组(P<0.001)。结论 术前检测乳腺肿块中端粒酶活性有利于良、恶性乳腺肿瘤的早期鉴别诊断和了解恶性乳腺肿瘤的发展状况,亦有利于乳腺肿瘤的早期治疗。

Clinical significance ofmeasurement of telomerase activity in human breast neoplasmas.

Abstract Objecteve Toinvestigate the difference of telomerase expression in human benign breast neoplasmas andmalignant breast cancer and its relationship to the pathological types, clinical TNMstages, growth and lymphatic metastasis of malignant breast cancer. Method Thetelomerase activity in fresh tissue preserved in liquid nitrogen was detected by usingtelomeric repeated amplification protocol silver staining assay. Results Thepositive rate of telomerase activity showed a significant difference between malignantbreast cancer and benign breast neoplasmas (P<0.005). No telomerase expression was foundin the adjacent noncarcerous tissues. Telomerase activity was increased gradually with thedeterioration of pathological types and the development of TNM stages. The telomeraseactivity in malignant breast cancer with the long diameter of the tumor ≥20 mm andlymphatic metastasis was higher than that in the group of the tumor diameter <20 mm andwithout lymphatic metastasis (P<0.001). Conclusion The detection ofthe telomerase activity in breast neoplasmas before operation might be helpful to earlydiagnose benifn breast neoplasmas and malignant breast cancer, to understand thedevelopment of malignant breast cancer and to early treat the breast neoplasmas.
