【摘要】 目的 调查幽门螺杆菌(Hp)感染与上消化 道癌危险性的关系。方法 在江苏省淮安市和邳州市，应用Hp IgG 定量酶联免疫(ELISA)检测试剂盒，随机检测312例上消化道癌及其健康一级亲属对照人群外周血中抗Hp IgG。结果 ①胃癌患者中Hp IgG抗体水平(阳性率50.0%)高于贲门癌患者和食管癌患者(P >0.05，χ² 检验)；贲门癌患者与食管癌患者之间，以上三类癌症病例与其自身亲属对照之间，差异亦不明显；②胃癌组病例与亲属同时感染Hp的阳性率(27.1%)显著高于贲门癌组和食管癌组(P <0.05)，后两组之间无明显差异。结论 Hp感染在胃癌家族中存在明显家庭聚集现象，Hp感染与胃癌危险性的关系有待进一步观察。

【Abstract】 Objective In order to analyse the associati on between Hp infection and the risk of upper - digestive tract c ancer. Methods In Huaian and Pizou cities, Jiangsu province, Hp IgG quan titative-enzyme-immunoassay methods was used t o identify IgG to H.p ylori in the serum of 312 cases of upper-digestive tract cancer patients and the ir kindreds. Results ⑴The level of IgG to H.pylori and t he IgG positive rate (50.0%) in gastric cancer patients were both higher than th at of cardia and esophageal cancer patients (P >0.05,χ² test) but no sign ificant differences were observed between cardia and esophageal cancer patients, 3 typies of cancer patients and their kindreds; ⑵the overall positive rates of both patients and kindreds in gastric cancer families (27.1%) were significantl y higher than that of cardia or esophageal cancer families (P <0.05) but no significant differences were observed between cardia and esophageal groups. Conclusion H.pylori infection was not thought to be correlat ed with the development of gastric cancer, although higher clustering of Hp infe ction in families afflicted with gastric cancer was noticed.
