【摘要】 目的 研究肠易激综合征 (IBS) 患者直肠对容量刺激的感觉。方法 应用电子气压泵及灌注导管测压仪研究20例IBS患者及20名正常人直肠对容量刺激的感觉、直肠顺应性及肛门直肠压力。结果 发现IBS患者肛门直肠括约肌静息压力为9.49±2.20 kPa(1 kPa=7.5 mmHg)，收缩压为15.34±4.29 kPa，松弛压为6.61±7.51 kPa，与正常人相似(P>0.05)；直肠顺应性亦无明显变化；但患者平均排便阈值及疼痛阈值分别为69.00±13.73 ml及123.00±32.62 ml，明显低于正常人的128.89±17.64 ml和217.78±42.95 ml(P<0.01)。结论 直肠对容量刺激过度敏感是IBS患者的重要表现之一，有助于解释IBS患者的部分症状。直肠对容量的感觉阈值可能为判断IBS疗效提供较好的方法。

     【 Abstract 】 Objective To investigate rectal visceral perception to balloon distention in patients with irritable bowel syndrome (IBS). Methods Rectal visceral perception thresholds to balloon distention, rectal compliance and anorectal manometry were examined in 20 patients with IBS and 20 healthy volunteers by means of electric barastat and perfusion catheter manometer. Results In patients with IBS, the anorectal resting pressure, squeeze pressure, and relaxation pressure were 9.49±2.20 kPa, 15.34±4.29 kPa, and 6.61±7.51 kPa seperately, which were similar to those of normal volunteers (P>0.05); rectal compliance had no obvious changes; while the average defecation thresholds and discomfort thresholds were 69.00±13.73 ml and 123.00±32.62 ml, which were much lower than those of 128.89±17.64 ml and 217.78±42.95 ml in normal volunteers (P<0.01). Conclusion The results suggested that hypersensitive rectal visceral perception to balloon distention was one of the significant manifestations in patients with IBS, which might help us to explain some symptoms. The thresholds of rectal visceral perception to balloon distention might provide a method to assess the treatment of IBS.
