输卵管妊娠腹腔镜线形切开术89例临床研究

【摘要】 目的 探讨输卵管妊娠腹腔输卵管线形切开术的优点及其对生育的影响。方法 将我院1998年1月～2003年6月281例输卵管妊娠患者随机分为腹腔镜手术组(A组)，开腹手术组(B组)，药物保守治疗组(C组)。分别对比分析A组与B组的手术时间、术后排气时间、离床活动时间、术后药物止痛、术后病率、住院天数；对比分析A组与C组的住院天数、治疗成功率、HCG转阴时间、重复异位妊娠、持续异位妊娠、输卵管通畅率、随访6个月及1年的妊娠情况。结果 A组与B组比较，术后病率、术后排气时间、离床活动时间、术后药物止痛差异有非常显著性；A组与C组比较，住院天数、治疗成功率、持续异位妊娠、HCG转阴时间差异有显著性。结论 腹腔镜输卵管妊娠线形切开术具有创伤小、恢复快、粘连少、住院时间短、治疗成功率高、增加术后受孕率等特点。对有生育要求的患者行腹腔镜输卵管线形切开术可提高生育机会。术中加用MTX介入治疗，可预防持续异位妊娠的发生。【关键词】 腹腔镜；线形切开术；输卵管妊娠；生育机会

【Abstract】 Objective The aim of this study was to analyze the advantages and the effect of fertilization by laparoscopic linearity split surgery in treatment of tubal pregnancy.Methods Through the two hundred eighty one tubal pregnancy cases between January 1998 and June 2003,divide into three groups.The group A was laparoscopic linearity split surgery group.The group B was paunching surgery group.The group C was medical keeping remedy group.Compared and analyzed group A with group B in surgery time,exhaust time after surgery,ovement time off bed,remedy acesodyne after surgery,incidence of illness after surgery,the number of days in hospital.Compared and analyzed group A with group C in the number of days in hospital,the rate of success cure,the time of HGG changing negative,repeating the different position pregnant,continuing the different position pregnant,flowing rate of fallopian tube,the situation later visited of six months and one year.Results Obvious difference in the incidence of illness after surgery,exhausting time after surgery,movement time off bed,remedy acesodyne after urgery.Group A and group C have obvious difference in the number of days in hospital,rate of success surgery,continuing different position pregnant and the time of HGG changing negative.Conclusion The characteristics of laparoscopic linearity split surgery treatment tubal pregnancy has of little hurt,quick resume,little conglutination,short time in hospital,high rate of success cure,increasing the rate of pregnancy after surgery to those patients who have the demand of bearing,the laparoscopic linearity split surgery will increase the chance of pregnancy using MTX remedy during surgery will prevent the occurrence of continuing different position pregnant. ......
