腹腔镜与剖腹手术剔除子宫肌瘤124例的临床对比观察

【摘要】 目的 比较电视腹腔镜手术与剖腹手术在国内保守治疗子宫肌瘤的临床效果、住院时间及术后恢复情况。方法 经临床诊断为子宫肌瘤要求保留子宫的住院病例124例，其中电视腹腔镜手术组及剖腹手术组均为62例，记录阳性体征、术中发现及观察术后恢复情况。结果 腹腔镜手术组时间及术中出血明显少于剖腹手术组[分别为(48±18)min比(68±27)min，(22±13)ml比(74±38)ml，P<0.01]，静脉用抗生素时间、肛门排气时间及住院时间明显短于剖腹手术组[分别为(2.66±2.19)天比(4.21±1.06)天，(1.21±0.46)天比(2.16±0.63)天，(4.62±3.03)天比(6.13±2.11)天，P<0.01]。结论 腹腔镜手术保守治疗子宫肌瘤具有手术时间短、术中出血少、术后胃肠功能恢复迅速、住院时间短等优点，值得推广应用。【关键词】 腹腔镜手术；剖腹手术；子宫肌瘤；比较；推广

【Abstract】 Objective To compare curing effect,the length of stay and functional recover of conservative treating to myoma of uterus between operative laparoscopy and laparotomy.Methods 124 cases of myoma of uterus that had been diagnosed by clinical and they were request reserving uterus and distributed to operative laparoscopy group(62 cases) and laparotomy group (62 cases).Their positive physical sign,detection in operation,length of stay were recorded.The postoperative recovery was observed.Results Operative time and bleeding in operative laparoscopy group were fewer than that in laparotomy group[(48±18)min vs (68±27)min,(22±13)ml vs (74±38)ml,P<0.01].The antibiotic time,functional recover time of bowel and length of stay in operative laparoscopy group were shorter than those in laparotomy group[(2.66±2.19)days vs (4.21±1.06)days,(1.21±0.46)days vs (2.16±0.63)days,(4.62±3.03)days vs (6.13±2.11)days,P<0.01].Conclusion Operative laparoscopy in curing myoma of uterus was better than laparotomy in operativing time,bleeding in operation,functional recover of bowel and the length of stay.It was worth of popularizing and using. ......
