初治空洞型肺结核介入治疗的研究

摘要：目的 探讨初治空洞型肺结核介入治疗的有效性和安全性。 方法 100例初治空洞型肺结核住院患者随机分为治疗组和对照组。治疗组在对照组应用常规抗结核治疗(2HRZE/7HRE)的基础上加经纤支镜引导灌注抗结核药凝胶介入治疗。 结果 临床观察治疗组总的痰菌阴转率为1000%, 2个月后痰菌阴转率和空洞闭合率分别为739%和696%,皆明显高于对照组的870%、478%和413%(P<005);且未发现与介入药物凝胶有关的毒副作用。 结论 在常规抗结核治疗的基础上加药物凝胶治疗初治空洞型肺结核疗效显著，其有促使痰菌转阴，空洞闭合的作用。且无明显毒性反应，并发症少，病人易于接受，值得临床推广应用。关键词：结核；结核杆菌;肺;空洞；介入治疗

Abstract:Objective To assess the significance of interventional therapy on cavitary pulmonary initial treatment tuberculosis of lungs Method Randomly selected 100 adult in house patients with cavitary pulmonary initial treatment tuberculosis of lungsSubjectswere divided into two groupsThe controlled group consists of 46 patients treated with traditional antiphthisic therapy had completed, 2HRZE/7HRE The therapeutic group consists of 46 patients treated with both traditional and the bronchoscope-assisted interventional antiphthisic therapy had completed Results For the sputum negative rate, the therapy group (100%) was higher than the controlled group (870 %) The cavitive closing rate was higher in therapy group (696%) than the controlled group (413%) too In addition, more clinically improvement had been shown in the therapy group than the controlled group There had not been any observed adverse reaction in the process of interventional therapy Conclusions Bronchoscope-assisted interventional antiphthisic therapy is an safe and effective way to treat refractory cavitary pulmonary initial treatment tuberculosis of lungs, provided the significant effect on cleaning, closing of the cavity as well as turning sputum negative Worthwhile, the clinical application should be approached extensively ......
