Superciliary arch-subfrontal drilling approach to

     Dr. Luan Limin of the department of neurosurgery, Shandong Provincial Hospital carried out resection of tumors of the sella turcica region through the superciliary arch-subfrontal drilling approach obtained good short-term results with less complications.

    On the basis of anatomical research, Dr. Luan Limin selected 32 cases with tumor in the sella turcica region consisting of the pituitary tumor, craniopharyngeal duct tumor, cholesteatoma and meningioma etc. Surgery was performed in all the cases through incisions inside the eyebrow or a little beyond the lateral side of the eyebrows. A mini-osseous fenestration was formed at the superior orbital region. The cerebral cisternae were exposed and dissected through an infra-frontal approach; the tumor was removed in piecemeal through microscope and endoscope. Postoperative follow-up was done for 19 cases (follow-up period of 2-8 months), and the results were compared with the control group. The results demonstrated that this surgical approach had similar exposing effect and those through the classical subfrontal approach and the pterion approach. During the follow-up, it was found that the patients were free of postoperative sequlae such as headache, diabetes insipidus, vision damage, loss of pituitary function, dysfunction of hypothalamus etc.. No recurrence of the tumors was seen during re-examination with CT or MRI. The operation through the mentioned approaches had a hidden incision, small bony fenestration, giving no cosmetic influence and also had the advantages of light trauma to the brain tissue, satisfactory exposure of the tumor lesion, a wide space of control and less postoperative complications.
