雌激素抗肾上腺素实验性心律失常作用

     摘要: 目的: 探讨雌激素在整体水平下的抗肾上腺素心律失常作用及可能机制。方法:24只新西兰大白兔随机分为对照组、利多卡因组及雌激素组,各组同步记录体表心电图及经左心室插管测定心功能指标[ 左心室内压(LVSP)、左心室舒张终末压(LVEDP)、左心室内压最大上升速率(+dp/dtmax)、左心室内压最大下降速率(-dp/dtmax) ], 比较各组按75 μg/kg 静脉注射0.1%肾上腺素前后心电图及相应心功能指标变化。结果:快速静脉注射肾上腺素可引起心功能指标明显上升,同时由于自律性增高致室性心律失常;雌激素可拮抗肾上腺素所致心功能改变,并延长室早(VP)出现时间,缩短室速(VT)持续时间及心律失常持续时间,其作用优于利多卡因(P<0.05)。结论: 雌激素对心律失常有一定对抗作用,可能与Ca2+阻断及改变细胞膜离子通透性有关。

    Effect of estradiol on experimental adrenaline induced arrhythmiaNIU Xinrong, Parhet·Abudureyimu, SHI Xinyan

    Abstract: Objective: To study the antiarrhythmia action and mechanism of estradiol on adrenaline induced arrhythmia. Methods: Twentyfour New Zea Land rabbits were randomly divided into three groups: control group, lidocaine group and estradiol group. The surface electrocardiogram (ECG) and cardiac function which was measured by inserting pipes into rabbits' left ventricle were simultaneously recorded in each group after infusion of 0.1% adrenaline with the dose of 75 μg/kg. The effect of adrenaline on ECG, cardiac function as well as arrhythmiagenesis were analyzed and compared between each group. Results: The cardiac function (LVSP, +dp/dtmax, -dp/dtmax)significantly elevate while LVEDP decrease after quick infusion of adrenaline, at the same time, adrenaline also induced ventricular arrhythmia due to increase autorhythmicity. Estradiol may antagonize the changes of cardiac function induced by adrenaline; furthermore prolong the latency of VP and shorten the duration of VT as well as arrhythmia, as a result, estradiol was prior to lidocaine (P<0.05). Conlusions: Estradiol could have the effects on antiarrhythmia, to some extent, have the relation with blocking calcium and changing permeability of cell membraneion. ......
