米非司酮对妊娠中期胎儿肾脏超微结构的影响

     【摘要】 目的 探讨米非司酮对妊娠中期胎儿肾脏超微结构的影响。方法 将中期引产孕妇随机分为两组:对照组(水囊组)10例(16~28周),试验组(米非司酮+水囊组)15例(16~22周)。试验组按孕龄分为:试验Ⅰ组(n=10,16~22周),试验Ⅱ组(n=5,23~28周)。试验组口服米非司酮100mg,连服3天,行水囊引产,对照组只行水囊引产。胎儿娩出后取肾脏组织进行样本制备,然后在电镜下观察组织的超微结构。结果 试验组肾脏组织的超微结构有改变,且孕龄越小,超微结构改变越明显:(1)肾小球毛细血管内皮细胞核形态基本正常,基质空化,线粒体肿胀;(2)系膜细胞核不规则,线粒体肿胀;(3)滤过膜内皮细胞胞质厚,无孔,基底膜薄,足细胞核基质空化,可见次级突起;(4)近曲小管上皮细胞核空化,胞质间可见空泡结构及较多溶酶体;(5)远曲小管大部分细胞核及胞质空化。结论 米非司酮可以引发胎儿肾脏的缺血缺氧性改变。

    Eeffects of mifepristone on the ultrastructure of fetal kidney in mid trimester of pregnancy

    【Abstract】 Objective To observe the effects of mifepristone on the ultrastructural of the fetal kidney in the second trimester pregnancy.Methods 25 normal pregnant women whose gestational age ranged from 16 to 28 weeks were randomly divided into 2 groups:control group(n=10,average gestation of 22 weeks)and experiment group (n=15,average gestation of 22 weeks).Control group was given water-bag induced labor alone.Experiment group was divided into 2 groups(experiment group Ⅰ:n=10,gestation of 16~22 weeks;experiment group Ⅱ:n=5,gestation of 23~28 weeks).Experiment group was administrated mifepristone 100mg orally per day for 3 days,then were given waterbag induced labor.After delivery of fetus,fetal kidney was separated immediately for observation by electron microscopic examination.Results Compared with the control group,the experiment group showed the significant ultrastructural changes of the fetal kidney and the earlier the stage the fetus was in,the more obvious the ultrastructural changes were:(1)glomeruluss cell nuclei was roughly eumorphism accompanied with matrix cavitation and mitochondria swelling;(2)mesangial cell nuclei showed irregular shape and mitochondria swelling;(3)filtraion membrane endothelial kytoplasm was thick,imperforated,basal membrane was thin with podocyte matrix cavitation and secondary ecphyma;(4)proximal convoluted tubule nuclei was cavitation,intracytoplasm was vacuoles and more lysosomes;(5)in distal convoluted tubule some nuclei and kytoplasm were cavitation.Conclusion Mifepristone could take ischemia hypoxia changes in the ultrastructural of the fetal kidney in the second trimester of pregnancy. ......
