摘 要：目的：研究包皮分层环切法的优越性。方法：自2000年12月至2004年12月应用分层环切法治疗包茎、包皮过长100例。平均年龄16.5岁。采用阴茎根部神经阻滞麻醉后行分层法包皮环切术，术中采用电刀剥离内、外板间的薄层组织。4～0肠线缝合切缘。缝线暂不打结，仅将两端缝线交叉；再在交叉缝线上扎紧直径约0.5cm与切缘等长的油纱条。结果：手术时间30～60min，平均37.5min。大部分病人线结可自行完全脱落；无切口局部感染；所有病例切缘整齐、美观；无疼痛性勃起或下弯畸形。结论：分层环切法具有切缘整齐，瘢痕形成少；术中出血少的优点。不易造成包皮切除过多或过少， 痛性勃起或阴茎上翘、阴茎头不能外露等畸形也少见，值得推广。

    Modified Foreskin Delamination Annular Shape Resection in 100 cases

    Patients with Redundant Prepuce and Phimosis

    Abstract： Objective： To study the superiority of modified foreskin delamination annular shape resection in treating redundant prepuce and phimosis. Method： 100 patients with phimosis and redundant prepuce were operated using foreskin delamination annular shape resection since December, 2000 to December, 2004. The average age of these patients was 16.5 years. Nerve block of Penile root before the operation. Thin-layer tissue between inner and external lamellae was detached by electric scalpe. Incisal edges was sutured by 4-0 catgut and then the end of the nesis being crossed but without being tied a knot. 0.5cm wide Vaseline carbasus strip as long as the incisal edge were use to tie the crossing nesis. Result： The operating time was 30 to 45min and the average time is 36.5min. Nesis knot detached itself in most of the patients. Incisal edge was smoothly and beautiful in all the patients. Aching erection and downward curvature deformation was not found. Conclusion： Delamination annular shape resection is an operating method with smooth incisal edge in foreskin resection. Furthermore, it also has the advantage of less bleeding during operation and minimal scar formation after operation. Other complication such as too much or too little foreskin resection, aching erection and curvature deformation are seldom. It is a good method to be spread. ......
