摘要：目的：研究剖宫产术后使用硬膜外自控镇痛(PCEA)的观察和护理措施。方法：硬膜外自控镇痛用于剖宫产手术，临床观察研究组和对照组共480例, 于术后72h内观察其镇痛效果及副作用的发生情况。结果：观察组术后6～24h镇痛有效率与对照组比较，镇痛效果差异极显著(P均<0.01)；观察组术后12h开奶并哺乳成功者及纯母乳喂养成功者均显著高于对照组( P < 0.01)；观察组术后肛门排气时间明显早于对照组(P<0.05)；观察组和对照组的恶心呕吐尿潴留发生率比较差异不显著(P均>0.05)。结论：剖宫产手术术后使用硬膜外自控镇痛，护理人员应密切观察，有针对性地进行护理，镇痛效果确切、安全，有效提高了术后的生存质量，有利于促进母乳喂养和术后的恢复。

    The Observation and Nursing of PCEA Applied to Analgesia after Cesarean Section

    Abstract： Objective： To discuss the observation and nursing project of patient controlled epidural analgesia (PCEA) applied to postoperative analgesia of cesarean section. Method： Patient controlled epidural analgesia (PCEA) is applied to cesarean section，totally 480 patients in the observation group and the control group. In 72h observes the effect of analgesic and the instance of untoward reaction after Caesarean section. Result： The observation group after cesarean section latter 6 and 24 hours analgesia effect there were significant differences compares with the contrast group, the analgesic effect discrepancy is very notable (P < 0.01);The observation group the colostrum within 12 hours after operations and succeed at pure breast-feed is higher apparently than those in the comparison group( P < 0.01).The anal exhaust time of patients is obviously early in the observation group than those in the control group(P<0.05)；The postoperative nausea and vomiting and postoperative urine retention is inapparent in the observation group than those in the control group( P>0.05 ). Conclusion： The analgesia after operations of cesarean section to use PCEA, the nurses should observe closely osculation,it is a safe and effective method for postoperative of cesarean section. it is advantageous the survival quality of postoperative, it is helpful to breast-feed to be advance and comeback. ......
