【摘要】 目的 探讨高龄癌症病人D2根治术的围手术期特点和管理方法。方法 回顾性分析我科2002年1月～2004年12月收治的136例70岁以上癌症病人D2根治术临床资料。结果 本组136例中，术后并发症发生率38.2%，围手术期死亡率1.5%。胸腔积液、肺部感染和吻合口漏是主要并发症。上腹部手术后胸腔积液的发生率较下腹部手术明显增加(χ2=13.6，P=0.0002)。结论 高龄癌症病人施行D2根治术是安全的，充分的术前准备和仔细的处理并发症是减少死亡率的关键。

    Experience of perioperative period management for D2 dissection in aged cancer patients

    【Abstract】 Objective To study perioperative period characteristics and management methods of D2 dissection in aged cancer patients.Methods The clinical data of 136 cases of cancer patients aged over 70 years after D2 dissection from January 2002 to December 2004 were analyzed retrospectively.Results In 136 cases performed D2 dissection,the complication rate was 38.2% and mortality rate was 1.5%.Major complications were pleural effusion,pneumonia and anastomotic leakage.Compared with hypogastric operation,the incidence of pleural effusion after epigastric operation was significantly increased(χ2=13.6 ......
