[摘要] 目的 探讨CT引导下经皮穿刺导管引流术在急性坏死性胰腺炎合并感染临床应用中的有效性和安全性。方法 经临床及影像学证实的急性坏死性胰腺炎合并感染的患者26例(Balthazar分级:Ⅰ级6例，Ⅱ级11例，Ⅲ级9例)在CT引导下行经皮穿刺导管引流术。26例患者共放置引流管38根。结果 12例患者(46%)仅行CT引导下经皮穿刺导管引流术治愈。9例好转(表现为感染症状得到控制，发热减退，白细胞恢复正常)。5例无效(患者穿刺引流后临床症状无明显改善而行外科坏死物切除术)。无一例患者出现与经皮穿刺导管引流术相关并发症。结论 CT引导下经皮穿刺导管引流术微创、靶向精确，并可有效控制感染(80.7%)，是治疗急性坏死性胰腺炎合并感染的有效和安全方法。[关键词] 体层摄影术，X线计算机；导管引流；胰腺炎

[Abstract] Objective To assess the efficacy and safety of percutaneous catheter drainage for treatment of infected acute necrotizing pancreatitis.Methods 26 patients (Balthazar gradeⅠ in 6 cases,grade Ⅱin 11 cases,and grade Ⅲ in 9 cases ) were performed with percutaneous CTguided catheter drainage for infected acute necrotizing pancreatitis shown with clinical and imaging findings.38 catheters were placed in 26 cases.Results 12 (46%) of the 26 patients were cured with only percutaneous catheter drainage,9 cases were improved ( Sepsis was controlled,defervescence of fever and return of WBC to normal).5 patients failed to show clinical improvement after drainage and required necrosectomy.No patient experienced catheterrelated complications.Conclusion Percutaneous catheter drainage is a safe and effective technique for treating infected acute necrotizing pancreatitis with microtrauma and accurate.It can control sepsis in 80.7% of patients. ......
