实时三维超声在产前胎儿大结构畸形筛查中的应用

     [摘要] 目的 探讨实时动态三维超声在产前胎儿检查中的应用价值。方法 对2680例16~28周的孕中期胎儿常规行二维和实时三维超声检查,仪器为GE Voluson 730 expert。实时三维成像方法应用经腹容积探头,在二维超声的基础上,采用表面成像法和透明成像法。将所采集图像存入硬盘,并对病变部位进行多方位扫描。最后对明确畸形诊断的胎儿,引产后进行尸解和染色体检查,对骨骼畸形的胎儿做X线检查。结果 2680例胎儿颜面部及肢体均能得到理想的二维和实时三维图像,其中发现畸形儿28例,无脑儿4例,胎儿多囊肾1例,重复肾1例,唇(腭)裂5例,骶尾部畸胎瘤1例,腹膜后寄生胎1例,四肢畸形3例,脑积水2例,腹裂1例,多发畸形9例,均经引产或生产后证实。结论 实时动态三维超声能更为详细全面且直观地提供胎儿形态信息及运动状况,是诊断胎儿大结构畸形,无创性产前诊断的首选方法。

    Real-time three-dimensional ultrasound imaging applied in deformities screening to prenatal fetuses

    [Abstract] Objective To assess the evaluation of real-time three-dimensional ultrasound in prenatal fetuses ultrasonography imaing.Methods Real-time three-dimensional ultrasound together with routine 2-D US was used to examine 2680 fetuses that were 16~28 weeks.A GE Voluson730 expert system was used.Volume-rendering images with surface and trans-maximum modes were used to reconstruct three-dimensional images.The regions of deformity were scanned multi-pointly and all images were stored in disks.At last the cadavers that were diagnosed as deformity were examined and examine chromosome or scanned by X-ray which had skeleton deformity.Results 2680 cases were imaged ideally,and all of them with deformities(28 cases),brain absence(4 cases),polycystic kidney disease(1 case) ......
