【摘要】 遵照 WHO 癌症三阶梯止痛原则，仍然可能有 10%~30% 癌症疼痛患者的疼痛无法理想缓解。对于难治性癌症疼痛的诊断，需要个体化分析诊断癌症疼痛病情，尤其需要注意动态评估疼痛程度及全面分析病情，认识神经病理性疼痛，认识复杂的区域性疼痛综合征。对于难治性癌症疼痛的治疗，需要个体化综合止痛治疗，尤其需要注意综合性抗癌治疗及支持治疗，突发性疼痛治疗，神经病理性疼痛治疗，腹腔内脏疼痛治疗，干预性止痛治疗等问题。

【 Abstract 】 According to the WHO cancer pain three ladder guidelines, there are still 10%-30% cancer pain patients may not be ideal to alleviate pain. For the patients with the refractory cancer pain, the cancer pain diagnostic analysis needs of individual patients, especially the need to pay attention to the dynamics of a comprehensive analysis and assessment of the degree of their pain, understanding of the neuropathic pain, and understanding of the complex regional pain syndromes. Instead of treatment for cancer pain, it is important to the individual integrated pain treatment, special individual anti-cancer therapy and support care, rational treatment of the breakthrough pain, the nuropathic pain treatment, and abdominal viscera pain treatment, intervention treatment of pain.
