胸苷磷酸化酶在胃癌的表达及与血管生成和预后的关系

     摘要: 目的 分析胸苷磷酸化酶(TP)在胃癌中的表达情况及其与患者年龄、性别、分化程度、淋巴结转移、肿瘤内微血管密度(MVD)及预后的关系,探讨TP的临床病理学意义。 方法 采用免疫组织化学SP法检测70例胃癌组织(患者术后均辅以以5氟尿嘧啶为主的化疗)及30例正常胃黏膜组织中TP的表达,并通过计算机图像分析法定量测定TP的水平;通过CD34免疫组织化学染色测定胃癌组织中的MVD。 结果 胃癌组织中的 TP水平高于正常胃黏膜;胃癌淋巴结转移组的TP水平高于无淋巴结转移组;胃癌组织中的 TP水平与MVD值呈正相关;胃癌组织中TP表达水平与患者的年龄、性别及分化程度无关;在浸润和转移情况一样时,胃癌TP高表达组的预后优于TP低表达组。 结论 胃癌组织TP表达与肿瘤的血管生成及淋巴结转移有关,并可作为患者对以5氟尿嘧啶为主的化疗疗效的判断指标。

    The Expression of Thymidine Phosphorylase in Gastric Carcinoma and Its Correlation with Angiogenesis and Prognosis

    ABSTRACT: Objective To study the relationship of thymidine phosphorylase(TP) with the degree of differentiation, lymph node metastasis, angiogenesis and prognosis of gastric carcinoma. Methods The expression of TP in 70 cases with gastric carcinoma and 30 cases with normal gastric mucosa was measured by immunohistochemistry and computer aided imaging analysis technique. Intratumoral microvessel density(MVD) was immunohistochemically detected using antiCD34 monoclonal antibody. Results The level of TP in gastric cancer cell was markedly increased than that of normal gastric epithelial cell(P<0.01). In addition, the cases with lymph node metastasis showed higher level of TP than those without lymph node metastasis. The level of TP in gastric carcinomas was association with microvessels density(MVD). The cases with high TP level had better prognosis than those with low TP level when the state of metastasis and infiltration was similarly. The level of TP showed no relation with age, sex and histological grades. Conclusion The expression of TP in gastric carcinoma is closely correlated with angiogenesis and lymph node metastasis. The expression of TP can serve as a efficacy index of FUbased chemotherapy in patients with gastric carcinoma. ......
