【摘要】 目的 观察中链甘油三酯(MCT)干预对2型糖尿病(T2DM)患者体重和体脂的影响。方法 将糖尿病患者分为MCT组、MCT/LCT组和LCT组，在接受食物交换份法的膳食宣教、总能量和脂肪摄入量得到控制的基础上，每天分别食用不同组成的食用油30ml，连续食用12周。在0和12周时进行膳食调查和评价。在0、6周和12周时测量空腹血糖、β-羟基丁酸、身高、体重、腰围和臀围，计算体质指数(BMI)和腰臀比(WHR)。 结果 在12周时MCT/LCT组摄取的总能量、蛋白质量和脂肪量比LCT组低，MCT/LCT组摄取的脂肪量也比MCT组低。在0、6和12周时，3组的空腹血糖和β-羟基丁酸差异均无显著性。相比LCT组，6周和12周时MCT组和MCT/LCT组的体重、BMI均较低，腰围也明显较小。12周时MCT组的WHR明显小于LCT组的WHR。 结论 在总能量和脂肪摄入量得到控制的基础上，用MCT取代100%或50%的膳食油脂，具有降低T2DM体重、腰围和腰臀比的减肥作用，对患者的空腹血糖和β-羟基丁酸没有影响。

    The effects of MCT on obesity in type 2 diabetes

    【Abstract】 Objective To observe the effects of MCT on body weight and body fat in type 2 diabetics.

    Methods T2DMs were divided into MCT group, MCT/LCT group and LCT group. With total energy and fat consumption controlled, they were given different proportion test oil after receiving education of food crossover method. All patients were told to take the test oil everyday for 12 weeks. We did diet survey and evaluation during 0 and 12 wk. We also detected fasting plasma glucose(FPG),β-hydroxybutyric acid(BHBA) and measured body height, body weight, waist line and hip line at 0 wk, 6 wk and 12 wk. Then BMI and WHR were analyzed.Results Compared with LCT group, total energy, protein and fat consumption in MCT/LCT group were decreased. MCT/LCT group also had lesser fat consumption than MCT group.There were no significant differences on FPG and BHBA among three groups. Compared with LCT group, body weight, BMI and wais tline of 6wk and 12 wk in MCT and MCT/LCT groups were decreased and WHR of 12 wk in MCT group was much decreased. Conclusion With total energy and fat consumption controlled, the daily diet oil is replaced full or partly by MCT, the body weight, BMI and WHR decrease on T2DM patients ......
