Medical Mystery — Visual-Field Defect

     A 31-year-old woman presented with a sudden onset of an inferonasal visual-field deficit in the left eye 2 days after she had traveled 16 hours by air. Physical examination on the day after the onset of her visual symptoms showed the findings in Panels A, B, and C. Auscultation revealed a systolic ejection murmur over the right upper sternal border. The hematocrit was 57.4%. What is the diagnosis?
