    Levosimendan for patients with impaired left ventricular function undergoing cardiac surgery

    eComment: We have been using levosimendan in more than 20 patients, where problems with weaning from bypass were expected. This comprised mostly emergency cases and surgery in patients with poor LV function. Ventricular arrhythmias did not appear to be a significant problem. We have the impression that the favourable circulatory effects of levosimendan cannot be explained by vasodilation and an increase in heart rate alone. However, I agree with Ibrahim [1] that the publication by Tokuda et al. [2] does not clarify the role of levosimendan in cardiac surgery.
