MVD\TSP-1在恶性血液病中的水平测定及意义(1)

     [摘 要] 目的 探讨恶性血液病患者骨髓中微血管密度(MVD)和凝血酶敏感蛋白-1(TSP-1)的表达及意义。方法 选择恶性血液病患者40例(19例急性白血病AL、8例慢性粒细胞白血病CML、13例多发性骨髓瘤MM)为研究组,正常对照组20例,利用SP免疫组化法检测研究组治疗前后及对照组骨髓组织活检病理标本中MVD水平及TSP-1表达情况。结果 ①MVD在AL、CML、MM等不同疾病中的同一阶段,测定水平无显著性差异(P >0.05);而在初诊组与未缓解组,MVD值高于对照组及缓解组(P <0.01)。②TSP-1在AL、CML、MM等不同疾病中的同一阶段,阳性表达无差异(P >0.05);而在初诊组与未缓解组,TSP-1阳性率低于对照组及缓解组(P <0.05)。③在MVD高表达组,TSP-1阳性表达率为21.2%,而MVD低表达组,TSP-1的阳性表达率为70.8%,TSP-1和MVD之间具有负相关性。结论 ①MVD是了解病情、判定疗效的独立指标。②TSP-1是血管生成负性调节分子。③MVD与TSP-1两者呈负相关,联合检测MVD和 TSP-1有助于更准确的评价疾病状态及疗效,指导治疗。

    SIGNIFICANCE OF MVD AND TSP-1 EXPRESSION IN MALIGNANT HEMATOSIS

    [Abstract] Objective To investigate the expression of bone microvascular density(MVD) and thrombospondin-1(TSP-1) in m alignant hematosis. Methods 40 cases (including 19 acute leukemia, 8 chronic myelocytic leukemia and 13 multiple myeloma) in study group and 20 cases in normal control group were analyzed by SP immunohistochemical technique to detect the expression of TSP-1 and the level of MVD in bone marrow biopsy. Results ①MVD expression levels in the same state of AL、CML、MM were no significantly difference(P>0.05), but MVD expression levels in newly diagnosed group or non-remission group were higher than remission group or control group(P<0.01). ②TSP-1 positive rate the same state of AL、CML、MM was no significantly difference(P>0.05), but TSP-1 positive rate in newly diagnosed group or non-remission group was lower than remission group or control group(P<0.05). ③In group of high MVD, TSP-1 positive rate was 21.2%. In group of low MVD level, TSP-1 positive rate was 70.8%. The expression of TSP-1 was negatively correlated with that of MVD. Conclusion ①MVD could be an independent index for the knowledge of state of disease and the judgement of efficacy. ②TSP-1 was negatively regulating molecule of angiogenesis. ③TSP-1 and MVD expression was negatively correlated. Combinable detecting of MVD and TSP-1 could help to estimate state of disease and judgement of efficacy more exactly and could direct therapy.
