【摘要】本文论述了高校医院开展健康管理的理论与实践。探讨如何开展健康管理和慢性病干预的对策和做法：收集个人完整的健康信息；综合个人情况对危险性进行评估，同时对危险因素采取干预措施，包括生活方式、膳食营养、心理健康、药物治疗等进行个体化指导；定期进行跟踪观察及效果评价，并制定未来的保健计划等。文章还针对存在的问题提出建议。
