不稳定型心绞痛患者血浆B型利钠肽变化及临床意义(1)

[摘要]目的探讨B型利钠肽水平与心绞痛分型、冠状动脉累及严重程度的关系以及治疗前后的动态变化。方法选择患者66例,其中不稳定型心绞痛患者26例,稳定型劳累性心绞痛患者20例,对照组20例。所有这些病人均予以冠状动脉造影、心脏超声、运动心电图及动态心电图的检查,治疗前后分别测量病人血中B型利钠肽浓度。结果 B型利钠肽的水平在不稳定型心绞痛组中较稳定型劳累性心绞痛患者及对照组患者的明显升高(44.6 ± 28.4 pg/ ml vs. 16.9± 9.2 pg/ ml p<0.01)(44.6 ± 28.4 pg/ ml vs. 10.8± 6.9 pg/ ml p<0.01)。B型利钠肽的水平在后两组患者中无显著性差异(16.9± 9.2 pg/ ml vs. 10.8± 6.9 pg/ ml p>0.05)。经过系统治疗后,不稳定型心绞痛病人B型利钠肽明显下降(44.6±28.4 pg/ml vs. 17± 9.1 pgs/ ml p<0.01)。不稳定型心绞痛组GENISINI评分与BNP有明显的正相关性(r=0.424p<0.05).稳定型心绞痛组BNP与GENISINI评分之间(r=0.384 p>0.05)无明显相关性。结论在不稳定型心绞痛患者中,B型利钠肽的水平明显升高,而且在规范治疗后B型利钠肽水平向正常水平恢复。血浆B型利钠肽水平与急性心肌缺血的范围有关。

Study on changes and its significance of B-type brain natriuretic peptid with unstable angina pectoris

[Abstract]ObjectiveToinvestigate the changes and clinical significance of B-type plasma brain natriuretic peptid (BNP) Levels in patients with unstable angina. MethodsWe measured the plasma levels of BNPin 26 patients withunstable angina,20 patients withstable exertional angina,and 20 control subjects. Coronary angiography、echocardiographic examination 、Holter monitoring andtreadmill test was performed at the time of blood sampling in these patients respectively. Results1.The plasma levels of BNPwere significantly increased in patients with unstable angina compared with those in patients with stable exertional angina and control subjects,respectively. (44.6±28.4 pg/ml vs. 16.9±9.2 pg/mlp < 0.01)(44.6±28.4 pg/ml vs. 10.8±6.9 pg/ml p < 0.01).Furthermore,in patients with unstable angina, the plasma levels of BNP decreased significantly after the medical treatment(44.6±28.4 pg/ml vs. 17±9.1 pg/ml p < 0.01). Plasma BNP level positivelycorrelated with theGensini score(r=0.424p<0.05).ConclusionThe plasma levels of BNP areincreased in the majority of patients with unstable anginaand that the increased levels decreased toward normal after treatment.
