牙齿重度磨耗伴牙列缺损咬合重建固定义齿修复的临床效果观察(1)

     摘要：目的 研究分析牙齿重度磨耗伴牙列缺损咬合重建固定义齿修复的临床效果。方法 选取我院2015年6月～2016年3月收治的牙齿重度磨耗伴牙列缺损患者56例，按照治疗的先后顺序分为对照组和应用组，每组患者28例。对照组患者实施牙合垫式可摘局部义齿修复，应用组患者实施固定义齿修复治疗，对比两组患者治疗后的咀嚼变化、美观度等。结果 两组患者修复后，应用组颞下颌关节功能改善的情况明显优于对照组。结论 两种方法都具有良好的修复效果，而重建固定义齿的修复效果具有美观度高、舒适性强，基牙继发龋发生率较低等众多特点，对患者改善颞下颌关节功能和改善咀嚼功能以及提高患者的生活水平的意义非常重要。

    The Clinical Effect of Fixed Denture Repair for Severe Tooth Wear Associated with Dentition Defect

    Abstract：Objective To study the analysis of severe tooth wear and defect of dentition occlusal reconstruction of the clinical effect of fixed denture.Methods In our hospital in June 2015 to March 2016 for severe tooth wear with dentition defect in patients with 56 cases， according to the order of treatment were randomly divided into control group and application group，28 cases per group.Patients in the control group received occlusal splint removable partial dentures，and the patients in the application group were treated with fixed denture restoration.The masticatory changes and beauty of the two groups were compared.Results After the restoration of the two groups，the improvement of temporomandibular joint function in the application group was significantly better than that in the control group.Conclusion The two methods have good repair effect，and the restoration of fixed denture has many advantages，such as high aesthetic，comfortable and low incidence of secondary caries.It is very important for patients to improve temporomandibular joint function and masticatory function，and to improve the living standard of the patients.

    随着人们生活质量的不断提高，对口腔牙齿的健康关注度也逐渐上升。临床口腔内表现为颌骨位置关系发生改变、曲线和平面的位置异常、硬组织(牙体)形态损伤、咀嚼肌行运动时和下颌关节功能改变等，常伴有牙列缺损(缺失)的情况发生，会对该类人群的生活造成一定的影响[1]。本文主要研究分析牙齿重度磨耗伴牙列缺损咬合重建固定义齿修复的临床效果，特选取我院2015年6月～2016年3月收取的牙齿重度磨耗伴牙列缺损患者56例，进行对比分析，取得了满意的效果，现报道如下。

    1 资料与方法

    1.1一般资料

    选取我院2015年6月～2016年3月收取的牙齿重度磨耗伴牙列缺损患者56例，按照治疗的先后顺序分为对照组和应用组，每组患者28例。对照组男15例，女13例，年龄57～79岁，平均年龄(65.3±4.2)岁；应用组男14例，女14例，年龄59～77岁，平均年龄(63.8±4.9)岁；所有患者符合以下各种情况：①牙齿重度磨耗并且牙本质牙面暴露>30%，切缘和牙合骨丧失釉质；②牙列缺失或缺损，并且连续缺失牙齿的数量≤2颗(除后牙游离端缺失)；③无牙周疾病。所有患者中有35例患者存在颞下颌关节病(TMD)。

    1.2方法

    1.2.1修復前准备 对患者口腔内的情况进行详细的检查，尤其是两侧颞颌关节需要通相关检查(X线)曲面断层及关节薛位片；对其长牙进行磨伸、牙尖尖锐并对牙槽嵴吸收在根长1/3以内；对无明显的残根进行根管治疗，对牙体缺损的患牙填充；所有患者进行常规的全口清洁，并对牙周组织支持不足的牙进行针对性的治疗，制定模型，对修复前的颌位关系进行详细的记录。, 百拇医药(周继华)
