不同治疗时机行宫颈环扎术对宫颈机能不全患者的疗效及妊娠结局的影响研究(1)
摘 要：目的 總结不同治疗时机开展宫颈环扎术对于宫颈机能不全患者的治疗效果和妊娠结局所产生的影响，为临床选择恰当的手术时机提供参考。方法 回顾性分析2016年1月～2017年1月我院收治的100例宫颈机能不全患者的临床资料，根据治疗时机分为研究组和对照组，每组50例。研究组患者采用紧急手术治疗，对照组择期进行手术治疗，比较两组患者的妊娠结局和对产妇与新生儿所产生的影响。结果 研究组足月生产产妇多于对照组，晚期流产(20.00% vs 60.00%)与宫内感染率(18.00% vs 62.00%)低于对照组，差异有统计学差异(P<0.05)，研究组早产率对于对照组，组间差异无统计学差异(P>0.05)；两组患者住院时间、孕周延长时间以及新生儿评分对比，差异无统计学意义(P>0.05)。结论 临床中对于宫颈机能不全患者需要在孕早期开展择期宫颈环扎术治疗，但是对于孕中期患者需要开展紧急手术治疗，提高术后的监测措施，促进改善妊娠结局。

Study on the Effect of Cervical Ring Ligation in Different Time of Treatment on Patients with Cervical Incompetence and Pregnancy Outcome

Abstract：Objective To summarize the effect of cervical ring ligation on the treatment and pregnancy outcome of patients with cervical incompetence，and to provide reference for clinical selection of appropriate surgical timing.Methods The clinical data of 100 patients with cervical incompetence from January 2016 to January 2017 were analyzed retrospectively.According to the time of treatment，they were divided into study group and control group with 50 cases in each group.The patients in the study group were treated with emergency surgery，and the patients in the control group were treated with elective operation.The pregnancy outcome and the effect on the puerpera and newborn were compared between the two groups.Results The full-term parturient in the study group was more than that in the control group，and the rate of late abortion(20.00%vs 60.00%)and the intrauterine infection rate(18.00%vs 62.00%)were lower than those in the control group，there is a statistical difference between the differences(P<0.05).There was no significant difference in the preterm birth rate between the study group and the control group(P>0.05).There was no significant difference in the length of hospital stay， prolongation of gestational age，and neonatal score between the two groups(P>0.05).Conclusion Clinically，patients with cervical incompetence need to undergo elective cervical cerclage treatment in the first trimester of pregnancy.However，for mid-pregnancy patients，urgent surgical treatment is needed to improve postoperative monitoring measures and to improve pregnancy outcomes.
