间隙性气压并促通仪治疗脑损伤后综合征60例分折

摘要：目的：观察间隙性气压并神经肌肉促通仪治疗脑损伤后综合征的疗效,并与单用药物治疗者进行对比。方法： 将2007-02/2009-06无锡市第二人民医院收治的颅脑损伤患者120例分为综合治疗组和对照组各60例。对照组应用脱水及支持治疗等常规治疗,综合治疗在此基础加用间隙性气压并神经肌肉促通仪治疗,20d后观察疗效(治愈：恢复正常生活，CT示脑挫伤区已恢复或基本恢复正常；好转：有头痛、头晕，生活基本自理或部份自理，CT示局部有脑退行性改变或有脑室扩大；无效：生活需要他人照顾，尚有智能及运动功能障碍，CT示局部有脑退行改变或脑室扩大)。结果：120例患者全部进入结果分折。经20d治疗后观察组总有效率明显高于对照组(97%比63%)，且临床症状及体征都得到明显改善。结论：间隙性气压并促通仪治疗可有明显改善脑损伤后综合征患者的症状和体征，优于单用药物治疗。

Abstract:Objective:Observes the gap barometric pressure and the nerve muscle presses to pass the curative effect which the meter treatment brain damage synthesizes drafts,and with only uses the medicine curer to carry on the contrast.Methods：Damages 2007-02/2009-06 Wuxi second people hospital admitting skull the patient 120 examples to divide into the complex therapy group and control group each 60 examples.The control group applies treatment conventional treatments and so on dehydrates and supports,the complex therapy adds in this foundation with the gap barometric pressure and the nerve muscle presses to pass the meter treatment,after 20d observes the curative effect (the cure:Restores the normal life,CT shows the brain to dampen the area to restore or to restore basically normally;Change for the better:Has the headache,dizziness,the life takes care of oneself basically or the part takes care of oneself,CT shows has the brain recession change partially or has the ventricles of the brain expansion;Invalid:The life needs other people attendance,still had intelligent and the movement function barrier,CT shows has the brain recession change or the ventricles of the brain partially expands).Results：120 example patients all enter a result minute booklet.Observation group total effectiveness is higher than the control group obviously after the 20d treatment (97% to 63%),also the clinical symptom and the symptom all have the distinct improvement.Conclusion：The gap barometric pressure and presses to pass the meter treatment to be possible to have the distinct improvement brain damage to synthesize drafts patients symptom and the symptom,surpasses only uses the medicine to treat.

由于交通事故或工伤、坠落等原因所致的损伤的颅脑损伤发生率、致残率均很高，且以年轻者为多见，患者经神经外科治疗病情稳定后，往往出现以自主神经及精神活动紊乱为主要临床症候群及精神异常、神经心理障碍，常规药物治疗疗效欠佳。本科应用间隙性气压并神经肌肉促通仪治疗这类综合征，观察其疗效。
1 对象和方法
设计：非随机化同期对照观察。
单位：一所三级甲等综合性医院。
对象：选择2007-02/2009-06无锡市第二人民医院收治的颅脑损伤患者120例，男64例，女56例；年龄28～70岁，平均42.5岁；脑震荡40例，脑挫伤80例。纳入标准：全部病例均经CT或MRI证实，诊断符合颅脑损伤诊断标准[1]，患者知情同意。排除标准：生命体征不稳定者。按就诊序号分为观察治疗组和对照组各60例。
设计、实施、评估者，经过培训，非盲法评估。
干预措施：对照组：给予常规药物治疗，包括一般治疗、病因治疗，脱水及支持疗法，适时应用脑血管扩张药，神经营养药。
综合治疗组：①常规药物同对照组。②用美国产IT间隙性气压治疗仪,6只气囊分别置于双下肢进行固定,充气压力为(9.09±1.3)kpa,每腔持续加压时间12s,每次治疗40min,每日一次。③应用日本株式会研究所生产的神经肌电促通治疗仪TENS21治疗 ......
