音乐疗法对面部整形美容局麻手术患者生命体征的影响研究(1)
[摘要]目的：探讨音乐疗法对面部整形美容局麻手术患者生命体征的影响。方法：将80例整形美容局麻患者随机分为实验组与对照组，每组40例，对照组给予常规护理，实验组在常规护理的基础上进行音乐疗法，观察两组患者就诊当天、术前(进入手术室时)、术中(手术开始30min)、术后(手术结束30min)的生命体征变化及术后疼痛视觉模拟评分法(Visual analogue scale，VAS)评分变化。结果：两组患者就诊当天及术前血压、心率比较，差异无统计学意义(P>0.05)；而术中及术后实验组患者的血压、心率均低于对照组，差异有统计学意义(P<0.05)。实验组的VAS评分均低于对照组，差异有统计学意义(P<0.05)。结论：舒缓的音乐可以有效缓解面部整形美容患者的紧张情绪，稳定生命体征，利于手术的顺利进行，整体提高护理满意度。

Study on the Effect of Music Therapy on Vital Signs of Patients Undergoing Facial Plastic Surgery with Local Anesthesia

Abstract： Objective To explore the effect of music therapy on the vital signs of patients who underwent plastic or cosmetic facial surgery using local anesthesia. Methods 80 patients who underwent plastic or cosmetic facial surgery using local anesthesia were randomly divided into the experimental group (n=40) and the control group (n=40). The patients in the control group received regular care， while the patients in the experimental group received music therapy in addition to regular care. The changes of vital signs and postoperative visual analog scale (VAS) of the two groups were observed at the day before surgery， preoperatively (at the time before entering the operating room)， intraoperatively (30 minutes after the beginning of the operation) and postoperatively (30 minutes after the operation). Results There was no significant difference in blood pressure and heart rate between the two groups on the day of consultation and before operation (P>0.05)， but the blood pressure and heart rate of the experimental group were lower than those of the control group during and after operation (P<0.05). The VAS scores in the experimental group were lower than those in the control group (P<0.05). Conclusion Soothing music could effectively relieve the nervousness of the patients who undergo facial plastic or cosmetic surgery， stabilize vital signs， facilitate the operation procedures and increase the overall satisfaction of nursing.

隨着人们物质生活水平日益提高，得到满足，对美的追求日趋强烈，整形外科技术也得到了迅猛发展[l]。面部是人体中最暴露的部位，也是最受人关注的部位。音乐疗法是医学、心理学和音乐相结合的产物，通过倾听欣赏的方式，达到治疗疾病的效果[2]。研究表明，音乐疗法可以有效降低机体应激反应，减轻手术患者的紧张、烦躁情绪[3]，改善患者心率、血压、呼吸频率，在手术中的应用广泛，疗效确切。手术作为一种应激源，可导致患者产生焦虑反应[4]。大部分患者术前会出现不同程度的焦虑、紧张心理，特别是局麻手术患者，由于局麻手术药穿透性的局限，患者术中常感觉疼痛，加之手术是在患者清醒状态下实施，手术过程中，器械的传递声、电刀的烧灼声、医护人员的说话声都会对患者造成干扰[5]。对手术的恐惧以及处于手术室的隔离环境，往往导致患者术前心率、血压升高，不利于手术进行。本研究通过对整形美容局麻手术患者进行音乐干预，探讨其对心理状态的影响。, 百拇医药(张冬梅 宋凯丽 刘畅 王鹏 朱雄翔)
