AnA\AnuA\抗Sm抗体\抗ds DNA抗体测定在诊断SLE中的作用(1)

     [摘要] 目的:通过检测抗核抗体(AnA)、抗核小体抗体(AnuA)、抗Sm抗体、抗ds DNA抗体,探讨它们在系统性红斑狼疮(SLE)中的诊断价值。方法:检测120例SLE患者中的AnA、AnuA、抗Sm抗体、抗ds DNA抗体水平,AnuA采用ELISA法测定,抗Sm抗体采用免疫斑点法测定,抗ds DNA抗体及AnA采用间接荧光免疫法测定。结果:在检测的120例SLE患者中,AnA、AnuA、抗Sm抗体、抗ds DNA抗体的阳性率分别为97.5%、85.0%、42.5%、45.0%,在结缔组织中的阳性率分别为77.6%、1.0%、2.0%、3.0%。健康对照组中的阳性率分别为15.0%、0、0、1.0%。AnA、AnuA、抗Sm抗体、抗ds DNA抗体的检测对SLE患者的诊断特异性分别为22.4%、99.0%、98.0%、96.9%。结论:AnuA对SLE患者的诊断特异性最高;AnA对SLE诊断的阳性率最高,但其特异性最低,AnuA、抗Sm抗体、抗ds DNA抗体测定都可作为SLE诊断的可靠指标。

    Roles of AnA, AnuA, anti-Sm antibody and anti-ds DNA antibody in the diagnosis of SLE

    [Abstract] Objective: To explore the roles of antinuclear antibody (AnA), anti-nucleosome antibody (AnuA), anti-Sm antibody and anti-ds DNA antibody in the diagnosis of systemic lupus erythematosus (SLE) by detecting their levels. Methods: The levels of AnA, AnuA, anti-Sm antibody and anti-ds DNA antibody in 120 patients with SLE were detected. AnuA was detected by using ELISA determination, anti-Sm antibody was detected by using immune dot determination, anti-ds DNA antibody and AnA were detected by using indirect fluorescent immunoassay. Results: In the detection of 120 patients with SLE, the positive rates of AnA, AnuA, anti-Sm antibody, anti-ds DNA antibody were 97.5%, 85.0%, 42.5%, 45.0% respectively; the positive rates in connective tissue were 77.6%, 1.0%, 2.0%, 3.0% respectively; the positive rates in healthy control group were 15.0%, 0, 0, 1.0% respectively. The specificity of AnA, AnuA, anti-Sm antibody, anti-ds DNA antibody in the diagnosis of patients with SLE were 22.4%, 99.0%, 98.0%, 96.9% respectively. Conclusion: AnuA shows the highest specificity for the diagnosis of SLE; AnA has a high positive rate in the diagnosis of SLE, but its specificity is the lowest, AnuA, anti-Sm antibody, anti-ds DNA antibody can serve as reliable indicators for the diagnosis of SLE ......
