清化消肿方治疗糖尿病肾病阴虚湿热型的临床研究(1)

[摘要] 目的:评价清化消肿方治疗糖尿病肾病阴虚湿热型的疗效。方法:选择2型糖尿病肾病阴虚湿热型患者70例,随机分为治疗组和对照组。对照组给予常规降糖、降压治疗;治疗组在常规降糖、降压的基础上,给予清化消肿方,共治疗4周。观察两组患者治疗前后24 h尿蛋白定量、症状、血脂、血糖、肾功能变化以及有无药物不良反应。结果:治疗组降低24 h尿蛋白定量明显优于对照组(P<0.05),并能改善症状,降低血脂、血糖,改善肾功能,肝功能、血、尿、大便常规均无异常。结论:清化消肿方治疗糖尿病肾病阴虚湿热型有较好的疗效,未见不良反应。

Clinical research of therapy with Qinghua Xiaozhong Fang for the Yinxu Shire syndrome of diabetic nephropathy

[Abstract] Objective: To evaluate the therapeutic effect of Qinghua Xiaozhong Fang for the treatment of diabetic nephropathy patients with Yinxu Shire syndrome. Methods: The selected 70 patients were randomized into the treatment group and the control group. Patients of two groups were given regular western medicine. The treatment group was treated by western medicine combined with Qinghua Xiaozhong Fang. Both groups were treated for 4 weeks. Changes in urinary protein, symptoms, blood fat and glucose, kidney functions and adverse reaction were observed. Results: The anti-urinary protein effect of the treatment group was significantly superior to that of the control group (P<0.05), and Qinghua Xiaozhong Fang could also improve symptoms, decrease blood fat, glucose and improve kidney functions, with no abnormal changes in liver functions and routine examinations. Conclusion: Qinghua Xiaozhong Fang has a good effect in the treatment of diabetic nephropathy patients with Yinxu Shire syndrome.

糖尿病肾病(DN)为糖尿病并发的微血管病变,是糖尿病致死的主要原因之一,其死亡率占60%[1]。国外研究显示,糖尿病病程为10～20年者,50%合并糖尿病肾病[2]。近年来,随着病例的不断积累,笔者发现糖尿病肾病尤其是临床期患者,证型多表现为阴虚热盛与湿热困脾同时存在,归纳为阴虚湿热型,故据此笔者筛选药物组成清化消肿方,治疗2型糖尿病阴虚湿热型36例进行临床研究,现报道如下:
1 资料与方法
1.1 一般资料
本组70例全部为2006年11月～2009年10月山东中医药大学第二附属医院肾脏病中心门诊和住院患者。治疗组36例,其中,男22例,女14例;年龄41～67岁,平均53岁;病程9～19年,平均11.4年;临床DN病程3个月～5年,平均2.1年;合并高血压19例,合并视网膜病变34例,合并周围神经病变15例。对照组34例,其中,男24例,女10例;年龄43～69岁,平均54岁:病程11～20年,平均12 ......
