基层医院妊娠期糖尿病管理模式探讨(1)

【摘要】 目的 探讨我院对妊娠期糖尿病孕妇的管理模式。方法 对2005年1月至2008年12月的妊娠期糖尿病孕妇的妊娠及分娩结局进行回顾性分析并和4年来所有分娩的母婴结局进行比较。结果 妊娠期糖尿病的发生率为4.74%。妊娠期糖尿病剖宫产率、早产率、巨大儿发生率高于平均值,其他母婴结局与正常孕妇无明显差异。结论 孕24~28周对所有孕妇进行妊娠期糖尿病筛查,对妊娠期糖尿病孕妇饮食控制、监测血糖及密切胎儿监测,妊娠期糖尿病适时终止妊娠可获得和正常孕妇接近的母婴结局。

【Abstract】 Objective To study the management mode of gestational diabetes mellitus of our hospital.Methods Using retrospective analysis to study the pregnancies of gestational diabetes mellitus and the following outcomes of our hospital from January 2005 to December 2008.And compareing with all pregnancies and outcomes of our hospital in the same period.Results The incidence of gestational diabetes mellitus was 4.74%.Cesarean section 、premature birth and macrosomia rate of gestational diabetes were above the rate of average.But other maternal and child outcomes with no significant difference between the normal pregnant women.Conclusion Gestational diabetes screening all pregnant women between 2428 weeks and with gestational diabetes diet control,monitoring of blood glucose andfetal monitoring,timely termination of pregnancies of gestational diabetes mellitus.The pregnancies of gestational diabetes mellituscan get similar maternal and fetus outcomes with normal pregnant women.

妊娠期糖尿病是指妊娠期出现或发现的的糖尿病。近年有逐渐增加的趋势,因糖尿病孕妇的临床经过复杂,对母婴均有较大危害,因此需加强孕产期的母胎监测,确保母婴安全。现对我院对妊娠期糖尿病孕妇的管理及妊娠结局进行回顾性分析,探讨适于基层医院糖尿病孕妇的管理模式。
1 资料与方法

年间我院分娩总数 15326例,发现妊娠期糖尿病 726例。妊娠期糖尿病的诊断标准参考乐杰主编的妇产科学第六版[1]。现将726例妊娠期糖尿病孕妇(GDM组)的妊娠经过及分娩结局及胎、婴儿情况与我院4年分娩(对照组)的母婴结局进行比较。统计学处理采用χ2检验。
2 结果
2005年1月至2008年12月妊娠期糖尿病发生率4.74%,其中妊娠期糖耐量受损201例,占 27.7%,糖尿病合并妊娠43例,占4.3%。产妇年龄21~45岁,平均28.9岁,初产妇456例,经产妇270例。
