     ANAHEIM, California (Reuters Health) - A study of a filter device used during angioplasty and published in the November 12th issue of Circulation: Journal of the American Heart Association yields two findings. The first is that the device, consisting of a collapsible filter placed distally during angioplasty, effectively collects significant amounts of debris released during the intervention; the second is that more such emboli are released than previously realized.

    The findings were released by the journal a day early to coincide with the 2001 Scientific Sessions of the American Heart Association that are being held here this week.

    Dr. Eberhard Grube of Heart Center Siegberg in Germany and colleagues there and at Stanford University in California, the University of Minnesota in Minneapolis, and the Cleveland Clinic Foundation in Ohio tested the AngioGuard Emboli Capture Guidewire (Cordis) in 26 patients who required angioplasty. Of those, 15 patients had occluded native arteries and 11 patients required intervention in saphenous vein grafts.

    The technical success rate was 96.2% and no major adverse events occurred during the study, Dr. Grube reports. The collapsible basket filter, placed downstream during angioplasty, allowed constant perfusion during the intervention, although the amount of debris collected in four patients was significant enough to temporarily block blood flow.

    Dr. Grube said that the usual procedure involves stent placement. He told Reuters Health that his team has had no difficulty in pulling the filter through the stent, and that the filter has not damaged any vessels.

    The researchers believe the filter device is particularly effective for patients with occluded saphenous vein grafts. This group has a 10-fold higher risk of adverse events caused by embolization during angioplasty of the grafts than their counterparts with native arteries.

    "This will become the standard of care with angioplasty," Dr. Grube declared.
