激素替代疗法不能完全防止骨质疏松

    NEW YORK (Reuters Health) - Hormone replacement therapy is thought to help prevent bone loss due to aging when taken after menopause. However, new study findings show that at least some women continue to lose bone when taking hormones.

    About 5% of women on hormone therapy lost at least 1% of bone mass in the lower spine within the first year of treatment, and 15% of women lost at least 1% of hip bone mass. During the second and third years, 8% of women lost spinal bone mass, while 12% lost hip bone mass.

    The study included 538 women taking hormones and 132 women who were not taking any hormones, according to the report in the November 13th issue of the journal Archives of Internal Medicine.

    But such bone loss is rare and does not necessarily mean that the treatment has failed in these women. It is possible that they would have lost even more bone mass if they had not started taking hormones, according to the team of researchers led by Dr. Gail A. Greendale, of the University of California, Los Angeles.

    It has been thought that some women lose bone mass while taking hormone replacement, but the estimates, ranging from 10% to 30% of women, have been informal, according to the researchers.

    Overall, older age was the most important risk factor for losing bone while on hormone replacement therapy. The researchers report that there appeared to be two groups of women who lost bone mass--those who lost bone mass during the first year and those who lost during the second and third years. Few women lost bone during both periods.

    Not surprisingly, many of the women not taking hormone therapy lost bone, but about 40% of these women did not lose any bone mass during the study, the authors point out.

    The study has several important implications, according to Greendale and her colleagues. They note that bone loss is rare in relatively young women who take hormone therapy after menopause.

    The team also points out that whether a woman gains or loses bone mass during the first year of taking hormone replacement therapy is not a good indication of whether she will continue to gain or lose bone. Some women stop losing bone after the first year of treatment while others begin losing bone in the second or third year, they note.
