心外科回天有术 先心病儿存活增多

     路透社纽约健康消息-近年由于采取了分期手术治疗方式及改善了术后护理,患有先天性左心发育不全综合征的患儿外科手术后的存活率较前有很大的提高。研究人员在美国心脏协会主办的《循环》杂志11月增刊上报告了这一研究结果。

    左心发育不全综合征是先天性心脏病患儿出生第一年内最常见的死亡原因。这种综合征相当常见,发病率约为出生活婴的1/4,000~6,000。患儿通常需在新生儿期接受首次外科手术,然后再在6个月及1到2年之间接受第二及第三次手术。手术主要是修复发育异常的左心室,它是心脏主要的泵血腔室。

    More Babies Survive Heart Surgery Than Ever Before

    NEW YORK (Reuters Health) - Infants born with a heart condition that is always fatal without surgery are surviving in greater numbers than ever before, thanks to a multistep surgical procedure and improved care after the operations, researchers report.

    The condition, hypoplastic left heart syndrome, is also the most common cause of death from congenital heart disease in the first year of life. The syndrome is fairly common, occurring in about 1 of every 4,000 to 6,000 live births.

    In an analysis of more than 800 infants who had been treated for the heart condition since 1984, Dr. Bernard J. Clark III, and colleagues at the Children‘s Hospital of Philadelphia, Pennsylvania, found that the number of children who survive the first operation has increased. The researchers report their findings in a November supplement of Circulation: Journal of the American Heart Association (news - web sites).

    The youngsters usually undergo surgery as newborns, again at 6 months, and a third time between 1 and 2 years of age. The operations repair the left ventricle, the heart‘s main pumping chamber, which is deformed in children with the condition.

    The survival rate after the first operation increased from 56% in 1984 to about 71% in 1998, with about 86% surviving today. The percentage surviving for 3 years, after all procedures are completed, has gone from 28% when the study began to 66% in 1998.

    ``I believe the improvements we‘ve seen in survival are due to several factors, including the addition of a second, intermediate operation in what used to be a two-step procedure, and improved management of the child after surgery,‘‘ Clark told Reuters Health.

    According to Clark, infants with hypoplastic left heart syndrome are almost always diagnosed within the first 24 hours of life, and can be placed on a medicine to treat them until they can be transported to a facility that performs the operations.

    ``It is critical that children with this heart condition be treated at centers where the operations are performed frequently,‘‘ Clark explained. ``I attribute a large part of our success to experience and the availability of a comprehensive cardiac center.‘‘
