    应用快速充盈期二尖瓣口间距与舒张末期

    左心室内径比值评估心功能的临床研究

    【摘要】 目的 探讨 M 型超声心动图测定快速充盈期二尖瓣口间距与舒张末期左心室内径比值

    ( E2E′ / LVDd)评估心功能的应用价值。方法 126例观察对象分为 A 组(正常组 , EF≥50 % ,65 例) 、 B 组

    ( EF为 30 %～50 % ,38例)和 C组( EF < 30 % ,23例) 。M型超声测定 E2E′间距、 LVDd ,计算 E2E′ / LVDd ;

    应用二维超声心动图采用双平面 Simpson 法测定 EF ;应用脉冲多普勒测定二尖瓣口与肺静脉血流频谱。

    结果 E2E′间距在 A组与B 组间差异无显著意义 ,但 E2E′ / LVDd在 A、 B、 C 3组之间的差异均有显著性意

    义( P < 0. 01) 。结论 在无瓣膜器质性病变时用 M型超声心动图 E2E′ / LVDd可以明确区分不同程度左

    心室收缩和舒张功能受损。

    【Abstract】 Objective To validate the new application value of M2mode echocardiography in the

    assessment of cardiac function with a novel index , the ratio of mit ral orifice distance durin g rapid filling phase

    ( E2E′ ) to left vent ricular (LV) end2diastolic dimension ( E2E′ / LVDd) , determined by M2mode echo in

    combination with the flow parameters of mit ral orifice and pulmonary vein derived f rom pulsed Doppler. Methods

    One hundred and twenty2six subjects were divided into three groups : group A (normals , ≥50 % of EF , n =

    65) , group B (30 % - 50 % of EF , n = 38) and group C ( < 30 % of EF , n = 23) . Fraction shortening (FS) ,E2E′ , LVDd and E 2E′ / LVDd were determined by M2mode echo , with sample line vertically penet rating septum ,mit ral leaflets and LV posterior wall f rom the parasternal LV long2axis view ; LV ejection f raction ( EF) was

    defined by two2dimensional echocardiography with bi2planar Simpson’ s rule ; The flow velocities of mit ral orifice

    and right upper pulmonary vein were recorded by pulsed Doppler f rom the apical 42chamber view. Re sults No

    marked difference was found in E 2E′between group A and group B ( P > 0. 05) . However , a significant

    difference was observed in E 2E′ / LVDd among group A , group B and group C ( P < 0. 01) . Conclusions In the

    absence of organic valvular diseases , E 2E′ / LVDd can differentiate the severity of impaired LV cont ractile and

    relaxation performances. The present findings show a new utility of M2mode echocardiography in the assessment

    of cardiac function.

    M 型超声心动图具有极高的时相分辨力 ,能够清

    晰地显示室壁运动及瓣膜活动曲线 ,可对房室瓣和半

    月瓣的开放与关闭速度、运动幅度以及射血时间等多

    项指标进行测定 ,从而推断瓣膜两侧压力的变化 ,此外

    也可通过测定等容收缩和等容舒张时间来判定心肌收

    缩与舒张功能[1 ]。本研究应用 M 型超声心动图测定

    快速充盈期二尖瓣口间距与舒张末期左心室内径比值

    (E2E′ / LVDd) ,结合二维和多普勒超声 ,探讨 M 型超

    声心动图在评估心功能方面新的应用价值。

    资 料 与 方 法

    一、研究对象

    本研究对象 126 例 ,分为 A、 B、 C 3 组。A 组为健

    (46. 33 ±18. 01)岁 ,均进行了健康体检、胸透、心电图

    和多普勒超声心动图检查无临床阳性体征发现 ,射血

    分数( EF) ≥50 % ......
