血浆同型半胱氨酸与冠状动脉病变支数的关系(摘要)

    目的 目的:探讨冠心病新的危险因素同型半胱氨酸(Hcy)与冠状动脉病变支数的关系。

    方法:采用高效液相色谱法对117例行选择性冠状动脉造影的患者进行了血浆Hcy水平的测定。冠状动脉造影显示三支冠状动脉中至少一支血管病变狭窄≥50%者为冠心病患者(n=101)。三支血管中任一血管狭窄程度均为0%者归入正常对照组(n=16)。冠心病患者进一步分为单支病变组(n=29)、双支病变组(n=35)及三支病变组(n=37)。

    结果:冠心病患者血浆Hcy水平显著高于正常对照组(17.56±1.14 μmol/L对10.59±0.43 μmol/L,P=0.02)。三支病变组及双支病变组血浆Hcy水平均不同程度地显著高于正常对照组(分别为19.49±1.98 μmol/L对10.59±0.43 μmol/L,P=0.006及17.41±2.03 μmol/L对10.59±0.43 μmol/L,P=0.036)。而单支病变组血浆Hcy水平仅有高于正常对照组的趋势(15.2±61.18 μmol/L对10.59±0.43 μmol/L,P=0.16)。

    结论:冠心病患者血浆同型半胱氨酸升高。血浆同型半胱氨酸增高越显著,则冠状动脉病变支数越多。进一步提示增高的血浆同型半胱氨酸是冠心病的危险因素。

    The Association between Plasma Homocysteine and the Number of Coronary Artery with Stenosis (Abstract)

    Objective: Previous studies have shown that elevated plasma homocysteine is a new risk factor for coronary atherosclerotic disease and is associated with the sxeverity of carotidartery stenosis and peripheral arterialdisease. In this study, we examine the precise relations of homocysteine to the number of coronary artery with stenosis.

    Methods: One hundred and seventeen subjects who underwent coronary angiography from July to October 1998 in our hospital were recruited in this study. Cases (n=101) were angiographically defined as those with ≥50% stenosis in at least one of the three coronary vessels, while control subjects (n=16) had 0% stenosis in any one of coronary vessels. Cases were divided into single-vessel group (n=29), double-vessel group (n=35) and triple-vessel group (n=37). Plasma homocysteine was measured within 1 week before anbgiography in all subjects.

    Results: Mean plasma homocysteine level was significantly higher in coronary patients than in controls (17.56±1.14 μmol/L versus 10.59±0.43 μmol/L, p=0.02). Homocysteine in triple-vessel group or double-vessel group was significantly higher compared with controls (19.49±1.98 μmol/L versus 10.59±0.43 μmol/L, p=0.006 and 17.41±2.03 μmol/L versus 10.59±0.43 μmol/L, p=0.036, respectively). There was only a trend to higher levels of homocysteine in single-vessel group compared with controls (15.2±61.18 μmol/L versus 10.59±0.43 μmol/L,p=0.16).

    Conclusion: Plasma homocysteine level is elevated in patients with coronary heart disease. The more increased homocysteine level is associated with more number of coronary artery with stenosis. Our results also suggested that the increased plasma homocysteine level is the risk factor of CAD., 百拇医药
