老年患者皮肤慢性溃疡的临床治疗研究

    [摘 要] 目的：总结老年患者皮肤慢性溃疡的修复特点。方法：分析了87例老年人皮肤慢性溃疡，对发病特点及修复方式进行探讨。结果：老年人特有的生理、病理特点决定了创面修复的复杂性，换药是一切治疗的基础，游离植皮是最安全、应用最广泛的手术方法；皮瓣转移适用于有深部组织暴露或疑有癌变患者；截肢适用于肢体远端高度疑有癌变或已引起全身淀粉样变者。结论：老年人皮肤慢性溃疡修复顺序的选择为换药治疗、游离植皮、皮瓣转移、截肢(指、趾)。

    [Abstract] Objective: To summarize the characteristics of the repair of chronic skin ulcer of the aged. Method: 87 old patients with chronic skin ulcer were treated and the characteristics of the pathogenesis and repair were discussed. Results: The physiological and pathological characteristics of the old patients make the repair of the ulcers rather complicated. Change of dressings is the basis of all treatment, free skin grafting is the most widely applied and safe method among the surgical operations skin flap transposition is suitable for treating deep tissue exposure and those suspected of canceration. Amputation is indicated for those with extremities cancer and systemic amyloid degeneration. Conclusion: The sequence of choice for treating chronic skin ulcer of the aged is change of dressings, free skin grafting, skin flap transposition and amputation (finger or toe). ......
