【摘要】 目的 为研究尿内皮素(UET)与糖尿病肾损害的可能关系和作用。方法 以放免法直接检测52例正常人、30例糖尿病、28例早期糖尿病肾病和25例糖尿病肾病患者UET排量。结果 各疾病组UET排量显著高于正常人组(P<0.01)。糖尿病组尿白蛋白排泄率<20μg/min时，UET排量已显著增高。UET与尿β₂ 微球蛋白排量显著性正相关(P<0.05)，与血浆内皮素水平、内生肌酐清除率无相关。结论 糖尿病患者UET水平随糖尿病肾病的严重程度而明显增高。UET排量增高可作为糖尿病早期肾损害的敏感指标。

    【Abstract】 Objective To study the possible relationship between urinary endothelin and diabetic nephropathy. Methods Urinary endothelin (UET) excretion was determined by redioimmunoassay in 30 diabetic (DM), 28 incipient diabetic nephropathic (IDN) and 25 diabetic nephropathic (DN) patients, and in 52 normal subjects who served as control. Results UET excretion in DM group (144.44±26.36ng/d), in IDN group (143.03±22.70ng/d) or in DN group (183.05±32.24ng/d) was statistically higher than that of normal subjects (59.19±9.96ng/d) (P<0.01).UET significantly increased even when urinary albumin excretion rate was below 20μg/min and correlated positively with the urinary β₂ -m(P<0.05). Conclusion The results suggest that increased UET excretion in DM patients is related to the severity of the diabetic renal lesion and could be an early and sensitive marker of diabetic nephropathy.
