【摘要】 目的 探讨肾上腺性征异常症的发病率、病因及诊治方法。 方法 报告肾上腺性征异常症12例，其中先天性肾上腺皮质增生(CAH)6例，肾上腺皮质肿瘤6例，结合国内文献报告共264例进行分析。 结果 CAH组中尿17KS升高，地塞米松抑制试验阳性，影像学示双侧肾上腺增大，治疗以皮质醇激素替代为主；肿瘤组中尿17KS明显升高，地塞米松抑制试验阴性，影像学示一侧肾上腺占位，治疗以切除患侧肾上腺肿瘤而愈。 结论 肾上腺性征异常征发现率明显增高，肾上腺肿瘤的发生率增高。通过年龄段、17KS值的高低、地塞米松抑制试验、影像学等资料分析有助于CAH与肾上腺皮质肿瘤、肾上腺皮质腺瘤与皮质癌的鉴别。

    【Abstract】 Objective To study themorbidity,etiology,differential diagnosis and treatment of adrenogenital syndrome. Methods 12cases of adrenogenital syndrome,3 male and 9 female,have been admitted and treated from1981 to 1996.There were 6 cases of adrenocortical adrenal hyperplasia (CAH) and 6 cases ofadrenocortical carcinoma.In addition,a total of 264 cases reported in China were reviewed. Results In6 cases of CAH,4 were masculization and 2 virilize prococious puberty.Increased17ks,positive dexmathasone inhibition test and bilateral adrenal hyperplasia on imagingprocedure were characteristic.In the 6 cases of adrencortical carcinoma,masculization wasnoted in all with negative dexamathasone inhibition test.On image examination,aspace-occupying mass was found in the adrenal gland in all the 6 cases. Conclusions Themorbidity of adrenogenital syndrome seems to be increasing because of better diagnosticprocedures and the constituent ratio of CAH and adrenocortical carcinoma is changing.17ksassay,dexamathasone inhibition test and image examination render great help to thedifferential diagnosis between CAH,adrenal adenoma and adrenocortical carcinoma.
