摘 要：目的 探讨肿瘤免疫逃避与膀胱癌发生、侵袭及复发的关系。方法 采用SABC免疫组化法检测14例膀胱腺癌(AC)，34例移行细胞癌(TCC)及10例慢性单纯性膀胱炎(SCC)石蜡切片中Fas配体(FasL)的表达情况。结果 SCC中FasL无表达；AC8例表达、TCC 20例明显表达，且与组织类型无关；FasL 的表达与临床分期及复发密切相关(P<0.05) ，而与TCC的病理分级关系不密切(P>0.1)。结论 FasL的异常表达参与了膀胱TCC的发展与复发，并影响其预后；FasL的表达异常与膀胱癌组织类型及病理分级关系不密切，其所致的肿瘤免疫逃避作用可能是恶性肿瘤发展的基本原因之一。

Abstract：Objective To clarify the relationship between tumor escape and the progression of bladder carcinomas.Methods Fas ligand expression was studied by means of immunohistochemical method in 10 cases of cystitis, 14 bladder adeno-carcinomas and in 34 bladder transitional cell carcinomas.Results None of the cystitis samples had FasL expression. The expression of FasL in AC and TCC were both signi-ficantly increased (P<0.05) and correlated with TCC stage (P<0.05) and recurrence (P<0.02), but not related to the tumor grade (P>0.1).Conclusions FasL might play an important role in the progression of bladder neoplasm. Tumor escape induced by FasL might be one of the basic reasons in progression of neoplasm.
