胃液固有荧光光谱对胃癌诊断价值的初步探讨

摘要 目的 探讨胃液固有荧光光谱在胃癌诊断中的应用价值。方法 胃液高速离心后,应用RF-5000荧光分光光度计,以250nm紫外光为激发光,测定300~800 nm发射光范围内的固有荧光光谱。结果 发现胃液荧光光谱有2个峰。胃癌患者与胃内良性病变患者胃液荧光光谱的峰位和峰数基本相同,不同的是胃癌患者第一峰(位于400nm处)的相对荧光强度指数(P1 FI)增大,而第二峰(位于420~430 nm之间)的相对荧光强度指数(P2 FI)相对减低。共测定了248例各种胃内良、恶性病变患者(其中进展期胃癌36例)的胃液荧光光谱,计算P2 FI/P1 FI,以该比值<2并且P1 FI>20.00为胃癌阳性判断标准,用于进展期胃癌的诊断,其敏感度为88.89%,特异度为84.95%。结论 胃液固有荧光光谱在胃癌诊断中具有广阔的应用前景。

A study on the autofluorescencespectrum of gastric juice in the diagnosis of gastric carcinoma Wang Jingtong, Lin Sanren, Zhou Liya, et al. The Third Hospitalof Beijing Medical University, Beijing 100083

Abstract Objective Toinvestigate the value of the autofluorescence spectrum of gastric juice in the diagnosisof gastric carcinoma. Methods Theautofluorescence spectrum of gastric juice after being super-centrifuged was scanned withRF-5000 spectrophotofluorometer when the excitation wavelength was 250 nm and the emissionwavelength ranged from 300 nm to 800 nm. Results The spectra of both benign and malignant gastric diseases werecharacterized by two peaks. However, the relative fluorescence intensity index of thefirst peak (P1 FI) at 400 nm of patients with gastric carcinoma was higher thanthat of patients with benign gastric disease; meanwhile, the fluorescence intensity indexof the second peak (P2 FI) between 420~430nm was relatively low. 248 patients with different gastric diseases including 36 patientswith advanced gastric carcinoma were collected in our study. The positive criterion forgastric carcinoma was composed of P2 FI/P1 FI<2 and P1 FI>20.00.The results showed that the sensitivity and specificity of gastric carcinoma were 88.89%and 84.95% respectively. Conclusion Theautofluorescence spectrum of gastric juice has a great prospect in the diagnosis ofgastric carcinoma.
